#ESMO26 Day 1 highlight for me: MASTERPLAN, now in Lancet Oncology. Radiation and pancreatic cancer have a complicated history. ESPAC-1 suggested adjuvant chemoradiation hurt. LAP07 and CONKO-007 couldn't show an OS benefit after chemo. A021501 closed its RT arm for futility. PREOPANC-2 was a draw. We walked through the adjuvant side of this story in Sem Rad Onc last year: doi.org/10.1016/j.semradonc.… So why is MASTERPLAN different? Phase 2, 14 sites in Australia and NZ (can i just say I love the Aus-NZ studies). 74 patients with high-risk localized PDAC, randomized 2:1 to chemo ± SBRT 40 Gy/5 after induction. 12-mo locoregional recurrence 20.2% vs 52.0%, sHR 0.44, p=0.022
PFS 11.9 vs 6.3 mo, HR 0.44
OS 22.0 vs 14.0 mo, HR 0.45
Grade ≥3 AEs 70% vs 64%. Why did this one work when others didn't? The rad oncs can explain the physics better than I can. I just read it as better tumor zap zap. Does it change practice? Maybe. I think we need to have a deeper conversation at every tumor board, but the juice seems worth the squeeze. @OncoAlert @TheGutOncLab @Onco_Nexus sciencedirect.com/science/ar…

A new dawn for pancreatic cancer is emerging. Two big approvals this year, multiple promising agents in trials, and now impressive early results with this combo. We still have a long way to go, but the momentum is real. More reasons for hope than ever. #ESMO26 @PanCAN @letswinpc

Phase III RASolute 302: Daraxonrasib Across Key Subgroups in Metastatic Pancreatic Cancer At the ESMO Congress 2026, Zev A. Wainberg presented additional exploratory subgroup analyses from the Phase III RASolute 302 trial (LBA82), evaluating daraxonrasib versus standard-of-care chemotherapy in previously treated metastatic pancreatic ductal adenocarcinoma (mPDAC). Key findings: 🔹 Overall OS (primary analysis): 13.2 vs 6.7 months (HR 0.40; 95% CI 0.30–0.53). 🔹 RAS genotype: OS hazard ratios numerically favoured daraxonrasib across evaluated subgroups, including patients without an identified tumour RAS mutation. 🔹 Preselected chemotherapy regimens: OS HRs ranged from 0.32 to 0.46. 🔹 Prior treatment setting: OS HR was 0.36 (95% CI 0.17–0.75) for early metastatic recurrence following perioperative therapy and 0.43 (95% CI 0.32–0.58) after one prior line of metastatic therapy. Takeaway: Exploratory analyses showed directionally consistent OS outcomes favouring daraxonrasib across clinically relevant subgroups. Findings in smaller mutation-defined subgroups require cautious interpretation due to limited sample sizes and wide confidence intervals. @MiteshBorad @FilippoPietran4 @ChiaraCrem1 @oncodaily @myESMO oncodaily.com/gastrointestin… #ESMO2026 #RASolute302 #OncoDaily #ESMO26

Excellent review in @CCR_AACR by @Dr_R_Kurzrock on an important non-RAS target in oncology, Claudin 18.2. Cancer adapts fast, so alternating between non-RAS and RAS targets in sequence may become an important strategy for maximizing survival. Yet non-RAS targets tend to fly under the radar these days. In pancreatic cancer in particular, Claudin 18.2 is important (for ~30% of patients); so is MEK (for almost all patients, and our focus at @Immuneering $IMRX with #atebimetinib), PRMT5 (for patients with MTAP deletions), CD73, GSK-3beta, and others. Have a non-RAS target in pancreatic cancer you think deserves more attention? Please add to the comments … @PanCAN @letswinpc #PancreaticCancer #PDAC My posts contain forward looking statements about Immuneering: please refer to our SEC filings for more information.
Another pan-cancer/tumor agnostic target. Claudin 18.2; 🎯🎯lots of nausea/vomiting though 🤢
