🧬 TroFuse-005 at ESMO 2026: Sacituzumab Tirumotecan Significantly Improves Overall Survival in Advanced Endometrial Cancer
Could a TROP2-targeted antibody–drug conjugate offer a new treatment option for patients with advanced or recurrent endometrial cancer after chemotherapy and immunotherapy?
At ESMO Congress 2026 Presidential Symposium III, Domenica Lorusso presented results from the phase III TroFuse-005 trial, evaluating sacituzumab tirumotecan (sac-TMT) versus chemotherapy in previously treated advanced or recurrent endometrial cancer.
🔬 Key Findings:
🔹 Median overall survival: 19.0 versus 12.1 months — a 6.9-month improvement (HR 0.68; p=0.0001).
🔹 Median progression-free survival: 7.2 versus 3.9 months (HR 0.59; p<0.0001).
🔹 Objective response rate: 35.1% versus 17.3%.
🔹 Duration of response: 11.1 versus 6.9 months.
🔹 Safety: Grade ≥3 treatment-related adverse events occurred in 54.4% versus 41.4% of patients. Stomatitis, anemia, and ocular surface toxicity were notable safety considerations.
Clinical Takeaway: TroFuse-005 demonstrated statistically significant improvements in both PFS and OS, supporting sacituzumab tirumotecan as a potential new treatment option after platinum-based chemotherapy and immunotherapy. Careful toxicity management remains important.

Phase II HRS-4642 ± Adebrelimab in KRAS G12D-Mutant Pancreatic Cancer
At ESMO Congress 2026, Chenlei Wen presented preliminary results from a Phase II study (LBA85) evaluating HRS-4642 plus gemcitabine/nab-paclitaxel (GnP), with or without adebrelimab, as perioperative treatment.
Key findings:
▫️ ORR: 63% vs 87%.
▫️ R0 resection: 100% in both cohorts among resected patients.
▫️ Major pathological response: 17% in both cohorts.
▫️ 6-month EFS: 87% vs 96%; survival data remain immature.
▫️ Grade ≥3 TRAEs: 47% vs 53%, with no fatal TRAEs or adverse events precluding surgery.
Takeaway: HRS-4642 plus GnP showed promising neoadjuvant activity. Adding adebrelimab yielded a numerically higher ORR, but its additional benefit remains unconfirmed.

Phase I C-CAR031 (AZD7003): Updated Results in Advanced Hepatocellular Carcinoma
At #ESMO26, Qi Zhang presented updated results from the Phase I study of C-CAR031 (1680RO), a GPC3-targeted armored CAR-T therapy, in 36 heavily pretreated patients with advanced HCC.
Key findings:
▫️ ORR: 44.4%; DCR: 91.7%.
▫️ Median PFS: 4.2 months; median DOR: 4.4 months.
▫️ Median OS: 18.0 months (95% CI 11.1–31.4).
▫️ 12-month OS rate: 65.4% (95% CI 47.1–78.7).
▫️ Safety: Grade 3 CRS occurred in 5.6%; no ICANS or dose-limiting toxicities were observed.
Takeaway: C-CAR031 demonstrated encouraging preliminary antitumour activity in heavily pretreated advanced HCC. The recommended Phase II dose was established at 2.5 × 10⁶ cells/kg, supporting further clinical development.

Phase III LEAP-012: Long-Term Overall Survival Results in Hepatocellular Carcinoma
At ESMO Congress 2026, Josep M. Llovet presented long-term results from the Phase III LEAP-012 trial (1678RO), evaluating lenvatinib plus pembrolizumab and TACE versus dual placebo plus TACE in unresectable, non-metastatic HCC.
Key findings:
🔹 Median OS: 40.4 vs 40.8 months (HR 0.98; 95% CI 0.76–1.26).
🔹 Median PFS: 14.6 vs 10.2 months (HR 0.70; 95% CI 0.56–0.89) in the updated descriptive analysis.
🔹 ORR (RECIST v1.1): 48.1% vs 33.7%.
🔹 Grade 3–4 treatment-related AEs: 70.5% vs 31.1%.
Takeaway: LEAP-012 met its primary PFS endpoint but did not demonstrate an OS benefit at the third interim analysis. The approximately 40-month median OS in both arms provides a new benchmark for future trials in unresectable, non-metastatic HCC.

Phase II OPTIMUM: Maintenance Olaparib ± Durvalumab in Advanced Biliary Tract Cancer
At #ESMO26, Changhoon Yoo presented results from the randomized Phase II OPTIMUM trial (LBA22), evaluating maintenance olaparib with or without durvalumab in patients with DDR-mutated advanced biliary tract cancer.
Key findings:
🔹 6-month PFS: 62.5% with olaparib + durvalumab vs 62.1% with olaparib alone.
🔹 Median PFS: 7.7 vs 8.2 months.
🔹 cHRD subgroup: 6-month PFS was 83.3% vs 53.5% in patients with other DDR alterations (p=0.008).
🔹 ctDNA: 6-month PFS was 88.0% with undetectable vs 44.1% with detectable ctDNA (p=0.002).
Takeaway: OPTIMUM did not meet its primary endpoint of a 69.3% 6-month PFS rate. Exploratory findings suggest more favourable outcomes in patients with cHRD alterations, supporting further investigation of HRD-targeted maintenance strategies.
