Most AI in healthcare is built by people who've never had a patient deteriorate on them at 3am. You can tell.

I'm hiring my next Chief of Staff. You won’t know every field I work in. What matters is how quickly you learn, the questions you ask, and whether you can think from first principles. You’ll help evaluate opportunities and work with founders building consequential companies. Technical background preferred. Three-year commitment. Menlo Park, in person. "If I had asked people what they wanted, they would've said 'faster horses'" Everybody in healthcare AI is building faster horses right now. But after we saw our 4 millionth patient, we could see we need to build something else, for the actual AI era we are entering. Now launching TORTUS HEALTH, a fully integrated, verticalised, AI native clinic. Talk to our propietary AI agent about any medical problem any time, reviewed and approved by a licensed clinician. This isn't a new product or feature, it's the start of a whole new model of care. Redesigned for you. Check it out at tortushealth.com

There is no such thing as motivation. There is only discipline. There is no such thing as discipline. There are only habits. There are no such thing as habits. There is only neurobiology. Cells that “fire together, wire together.” Once you understand the basic unit of why you do things, you will understand everything else.

TORTUS is extremely pleased to announce that we are now certified as a Class IIa UKCA Medical Device. For those who know what the journey from Class I (self-declared) to Class IIa involves, you can appreciate the blood, sweat and tears the team put in to get here. There really aren't words to fully express my admiration and pride in this team. For those who don't, here is the journey to Class IIa, by the numbers: 2 years of graft, end to end 284,000 words, equivalent to four full books 136 individual, legal-standard documents Hundreds of internal meetings 4 major external audits £1.5m in estimated resource And the infinitely harder to measure: the late nights, the missed family dinners, the early starts, the sacrifices that got us here. Years in the making, to give our clinicians and patients the assurance that TORTUS is there for them in the moment that matters. Read more about our journey to Class IIa here: lnkd.in/eKj--JEM #takeyourtimewithTORTUS #yearsinthemaking Huge thanks to our partner Scarlet and to the TORTUS team, past and present, for this phenomenal achievement. @matthewclifford @Alicebentinck @vkhosla @drhpaz

Prompting 101: - take the task you are trying to do - estimate how long it would take you or someone skilled to do it manually - take 5% of that time - spend that time crafting the prompt and context - you will thank me later

I don't really have any VC horror stories - @vkhosla led our Seed round and has been extraordinarily generous with his time, advice and support, even visiting our London HQ office when he was in town. I give the same advice to everyone that asks for a reference about VC funding: 1) Know your investors, know what they are looking for, and know the best will have the very highest of bars, which may not be for everybody. 2) They are humans too - get to know them. They are flawed, they are going to be wrong sometimes, you shouldn't always listen to them and be prepared to debate vigorously - that makes your company better, not weaker. 3) Most companies that changed the world are VC-backed, that isn't a coincidence. If you want to build something at generational scale then this is the way, but it's not a path for everyone.

The hardest skill to learn in life is letting someone close to you fail. Freedom to fail is the only way any of us a) learn and b) grow and c) achieve beyond our abilities.

If people don’t actively tell you you are being too hard on yourself, you are probably not being hard enough.

I love a crisis. Some people do, some people don’t. Hard to explain why, but IYKYK. 😂

The harder it gets, the bigger the challenge. The bigger the challenge, the funner the game. We all played S Level video games for a reason 😂

Most of optimising for investment does the opposite - if your business is on fire, it needs your attention, not investment. -> and if your business is ON FIRE 🔥 then investors will come to you. So either way it's probably a waste of your time.

Ernest Codman (1869–1940) was an American surgeon and early healthcare quality pioneer best known for creating the “End Result Idea,” which argued that hospitals and doctors should track patient outcomes and analyze errors to improve care. Despite this being written over 100 years ago, we fail to do this in most systems today. Despite decades of digitization, joined-up systems discussion, and integration, we haven't created a culture of learning from our mistakes as clinicians - in fact, quite the reverse. Whenever there is an SI, every single person in that chain of care should be notified so we can all learn. In the AI-first future we are moving towards, perhaps we will finally have Kaiser's 'Learning Healthcare System' we have talked about for a century.

You can’t control the output, only the inputs.

Clinicians transitioning into tech - don't underestimate how much of your insight is actually subconscious, entrained over years in the system. a) it takes a ton of examining your thoughts and challenging them when building new things b) you're going to have to overexplain every rationale to non-clinicians who haven't lived in that environment

For every 1 unit of energy in AI it takes to actually build the model. It takes 10 units to evaluate it. And 100 units to actually implement it. Factor this in when starting something new today. If it's going to take you 1 year to build it, it'll take 10 to eval it, and forget implementation. If you can build it in 1 month, plan to sell it by the end of the year, but you'll spend the next 8 years deploying it.
