I usually share the latest evidence. Many articles, however, can be impactful long after publication. I will start sharing some in a new feature: #AgelessGems💎. The first: a review by @GeorgeSledge51 on curing MBC. Thought provoking, balanced, unmissable. ascopubs.org/doi/full/10.120…

🔥 A 25-year paradigm challenged in early BC: lidERA shows a giredestrant, a next generation oral SERD, outperforming aromatase inhibitors iDFS HR 0.70 DDFS HR 0.69 This is really a breakthrough❗️ My take home on clinical implications 👉 High risk: CDK4/6-AI remains standard (more mature trial results). CAMBRIA 2 will define the role of CDK4/6-SERD combo 👉 Intermediate/high risk: SERD might be preferred (with more mature data) to CDK4/6. Is combo better? What about sequencing them (CAMBRIA1, EMBER4, ELEGANT)? 👉Intermediate risk: SERD preferred to CDK4/6

HER2CLIMB-05 shows addition of tucatinib to maintenance HP, after median 6 cycles THP, improved mPFS 8.6 mos to 24.9 mos. Benefit more pronounced in HR-, but HR+ benefits as well. #SABCS25 @ErikaHamilton9 @OncoAlert

A new chemo free maintenance option for patients with HR+ & HR- HER-2 + disease in the 1st line setting post induction chemo that can provide a meaningful benefit in excess of 2 years is a win for our patients! @SABCSSanAntonio @OncBrothers @OncoAlert @SarahCannonDocs
Adding tucatinib to standard maintenance therapy led to progression-free survival of more than two years in patients with HER2-positive metastatic breast cancer, according to results of the HER2CLIMB-05 trial presented by Dr. Erika Hamilton at #SABCS25 brnw.ch/21wYeG1

The billion dollar question: should we sequence ADCs for treating MBC? Proud to join @JoshDragoMD in this @NEJM debate, where we review the data in support and against the sequential use of ADCs. Open access — read it here: nejmgroup.org/wp-content/upl…

Replying to @ErikaHamilton9
@ErikaHamilton9 presents primary results from HER2CLIMB-05: adding tucatinib to 1L maintenance HP significantly improved PFS; most benefit in ER-/HER2+ disease (delta 12 mo). More diarrhea and liver tox with tucatinib. New SoC maintenance option after induction for ER-/HER2+ MBC!

In our retrospective Flatiron data presented at #SABCS24, T-DM1 did seem to outperform a second topo1 ADC after progression on prior T-DXd.
Could it be more useful t-dm1 after topo1 ?

Evolution of HER2 testing over 40 yrs. IHC lacks the dynamic range to predict outcomes with T-DXd, yet novel quantitative assays are emerging. We presented data with HS-HER2 to predict T-DXd activity at #ESMO24. More data coming at #ASCO25. Stay tuned. nature.com/articles/s41571-0…

We have good evidence that leaving lymph nodes behind does not necessarily compromise outcomes. Could we push the limit & leave the tumor unresected in some responders? Early days, but fascinating concept. Congrats to the authors for this important report! jamanetwork.com/journals/jam…

Really proud of my work wife (@itsnot_pink) filling the shoes of the late Anne Loeser - the driving force & founder of @the_rightdose. She leaves a legacy that improves the lives of MBC patients who are on treatment indefinitely through drug dose optimization. #SABCS2024 #bcsm

There is an ongoing trial (STOPHER2) to look at stopping tx in people with HER2+ MBC who have been NED for a while, and there has been a study looking at changes in ctDNA with time in ER+ but I’m not aware of one looking at stopping tx if ctDNA undetectable . #SABCS24
