Could include Occupational/Physio-Therapies as well, esp if any are NMP - works well if anyone needs equipment to facilitate d/c. I’ve seen some Trusts in UK have trialled joint Paramedic & Therapy triage/discharge in community with equipment or other prescription if identified.
