We tested over 30 new ways of calculating a CAC score to incorporate the inverse association between plaque density and risk and develop CAC-VD. CAC-VD demonstrated improved performance over the Agatston score in MESA and an external validation cohort.
Can we improve on the Agatston score? 🫀 The new CAC Volume & Density (CAC-VD) score is simple to calculate, accounts for the inverse association of calcium density with CHD risk, & improves risk prediction vs the Agatston score @hsbhatia @UCSDCards

Yesterday, we kicked off #NLAFall 2026 with one of the most important topics in lipidology right now: Lipoprotein(a).
Moderated by the incredible Marlys Koschinsky, PhD, FNLA, our faculty — Michael J. Wilkinson, MD, FNLA, Dinesh Kalra, MD, MBA, FNLA, and Christie M. Ballantyne, MD, MNLA — walked us through the fundamentals every clinician should know, the emerging therapies on the horizon, and what the future of Lp(a) management looks like.
Day Two is here, and the agenda is stacked. From the 2026 Dyslipidemia Guideline and the latest clinical trials to the 2025-2030 Dietary Guidelines and our Lipid Scholar case presentations, the science isn’t slowing down.
Here’s to an incredible day in Orlando! 🌴

Recent paper @JACCJournals in JACC: Advances from @FamilyHeartFdn - FIND Lp(a), a pragmatic, scalable, innovative ML approach to close an implementation gap. Despite recommendations for once-in-a-lifetime Lp(a) measurement, testing remains very low—even among patients with established ASCVD. Using data from 344,987 adults with ASCVD, the FIND Lp(a) model identified a group of 1553 adults in whom 55.1% had Lp(a) ≥125 nmol/L—more than doubling the yield of screening (2.2-fold enrichment). Enrichment was even greater at higher Lp(a) thresholds. Such ML models may help health systems move from recommendations to action. Next steps will be to prospectively validate and seamlessly integrate into real-world workflows without adding unnecessary burden to clinicians. This is my 10 point conversations, I tend to have with any patient who might have had a predetermined mindset based on social media, CHATGPT or influencers.
1. Based on your age/gender and risk, you are already a candidate for statins, if you choose CAC, there is a 40-50% chance that you may have #PowerOfZero and likely risk below threshold of why suggested statins, allowing you flexible decisions.
jacc.org/doi/10.1016/j.jacc.…
2. There is no other good tests that will give you the same information about derisking to guide you optimal information to derisk
ahajournals.org/doi/full/10.…
3. If decide to make decisions based on CAC vs current risk scores, likely will result in better adherence as seen in the yet to be published CorCal, CAUGHT CAD RCT’s as well accumulation of retrospective literature sciencedirect.com/science/ar…
jamanetwork.com/journals/jam…
4. Likely better LDL and risk factors control (EISNER and Caught CAD)
jacc.org/doi/10.1016/j.jacc.…
jamanetwork.com/journals/jam…
5. In a shorter term, CAC guided decisions will likely reduce the risk of plaque progression based on CAUGHT CAD
jamanetwork.com/journals/jam…
6. Thankfully if we can keep the price $150 or less or impact your disutility of taking pills it is even cost effective from a societal perspective
jacc.org/doi/10.1016/j.jcmg.…
7. In a clinical trial those randomized to decision based on CAC vs not CAC, we haven’t seen increased risk of extra stress tests, caths or stenting/bypass (EISNER)
jacc.org/doi/10.1016/j.jacc.…
8. While to date there is no data showing CAC guided decision will reduce the risk of CVD clinical events in a short term period, the studies have shown a) there is no increased risk (harm) of MACE (CorCal), b) prior non randomized studies have suggested no better outcomes with statin use, suggesting no harm if you decide to have flexible goals
jacc.org/doi/10.1016/j.jcmg.…
9. CAC is not one time test, if zero and don't go on treatment repeat in 3-7 yrs based on baseline risk estimate to guide if need to change the decision course
jacc.org/doi/10.1016/j.jcmg.…
10. Now based on studies, CAC beyond guiding you whether you need statin or not, can help as a dosing strategy (higher risk, higher intensity) now acknowledged also by cholesterol management guidelines

Khurram, respectfully, your reply perfectly captures the issues I have with this test. FROM THE PATIENTS POINT OF VIEW, they do not have coronary disease prior to the CAC. After the test, they have that diagnosis which they will Google and ask ChatGPT about. This will trigger a range of downstream actions ranging from taking a statin (good) to anxiety (bad) or unneccessary testing (very bad). So in my view, which matches the patient perpective, this is indeed a screening test. You may see it as a decision tool, but the patient and those outside medicine see it as a diagnostic/screening test for sure.

Outstanding post by @khurramn1 in getting correct message across from article. A PREVENT-ASCVD score of 5-10% should lead to Clinician-Patient Benefit/risk discussion. In a person not inclined 2 going on Lipid lowering Rx, a CAC 0 or a very low CAC % for age can lead to deferral
Highest respect for @EricTopol , but I think his message on CAC is slightly off target
We are still looking too much at summary prediction metrics and not enough at the actual clinical question
When someone is in the gray zone, does CAC help decide whether to start a statin?
Look under the hood
1. Among those with PREVENT risk 3–<5%, where statins are being considered, about 60% have CAC=0
2. Among those with PREVENT risk 5–<10%, where statins are recommended, about 40% have CAC=0
3. Their observed event rates are also below the risk levels where PREVENT would otherwise recommend treatment
4. And the reclassification results tell the same story. A large proportion of patients move below the treatment threshold once CAC is known
This is the information a patient and physician actually care about
If nearly half of people sitting in the statin decision zone can potentially defer therapy after CAC=0, isn’t that exactly the point of shared decision-making?
And CAC=0 is not a lifetime pass
@MichaelJBlaha and others have shown, and now endorsed by guidelines, a repeat CAC in 3-5 yrs in selected patients identify the few with CAC zero at risk, allowing to modify treatment plans
Also CAC is not just about starting statins...it can guide dosing strategy (as per guidelines)
On the other end, those with the highest CAC especially in intermediate risk, where one should pursue more intensive targets
I just wish we would spend less time on irrelevant metrics and more time asking the right clinical question and looking carefully at what the data actually show
How well can CAC change the treatment decisions?
isn’t that the whole spirit of shared decision-making?
