Check out this month’s #AJNR Fellows’ Journal Club choice: ‘’Sacral Dural Tears in Spontaneous Intracranial Hypotension: Imaging Phenotype and Treatment Outcomes in a Multicenter Cohort’’ Sacral dural tears are an often overlooked cause of spontaneous intracranial hypotension, with treatment outcomes primarily influenced by baseline imaging features and disease chronicity rather than procedural technique In this retrospective multicenter cohort, unorganized extradural CSF collections predicted better clinical and imaging outcomes, while epidural patching achieved complete clinical improvement in 56% of patients. Dynamic CT myelography outperformed digital subtraction myelography in leak localization (75% vs 31%). These findings highlight the importance of morohology based diagnosis and sacral-directed imaging and treatment. ajnr.org/content/47/10/2674 #neuroscience #radiology #radiologyresident #intracranialhypertension #sacralduraltears

After getting some great feedback from patients using Upright, I’ve just released an update with some improvements and new features: • Appointment preparation: put your story into words, prioritize your questions, and bring a printable brief to your visit • Upright/flat tracking: record total daily upright time • Updated PDF reports: see symptom trends over time alongside treatments and position tracking • Easier medication logging: reuse previous medications and enter doses from earlier dates The appointment feature grew out of conversations in clinic. People often arrive with a lot they want to ask and understandable worry about whether they’ll be heard. My hope is that this helps you bring what matters most into the conversation. Thank you to everyone who’s tried the app and shared feedback. Download Upright for free on the App Store: apps.apple.com/us/app/uprigh…

Several patients have asked whether this new “Neuro-EDS” paper explains their illness that I wanted to share my assessment. I rarely write threads like this. But patients deserve to know what an explanation establishes before it shapes their care. A spinal CSF leak perspective: 🧵 preprints.org/manuscript/202…

My work in spinal CSF leak makes me want this area to receive renewed attention and respect. I worry that stigma makes patients harder to hear and clinicians less willing to engage. We need serious investigation and room for disagreement, with continued care for patients.

Good science designs studies that could change our minds, including about explanations we favor. I want that standard applied to these proposed mechanisms and to my own skepticism. The goal is evidence that helps patients choose care likely to improve their lives.
