For many men with urinary symptoms from an enlarged prostate, medication is where treatment starts. Medication is great, but it’s important to understand what they’re doing because it’s not curing the underlying prostate enlargement.
If you’re going to take a medication for years, ask:
- What is this medication trying to accomplish?
- What side effects should I know about?
- What happens if it stops working well enough?
- What non-medication options are available if my priorities change?

There are a few things I wish more men knew about their prostate before symptoms become a major problem. The first is simple: Pay attention to changes in how you urinate. If you’re waking up more often at night or going more frequently during the day, bring it up with a doctor.

ALPFA’s GRAIL system uses short electrical pulses designed to target prostate tissue while sparing nearby structures. Why’s that interesting? Every BPH treatment involves tradeoffs: obstruction relief, recovery, bleeding, sexual function & the possibility of retreatment.

There’s a new group of urethral implants emerging for enlarged prostate treatment: Zenflow, Proverum, Butterfly, FloStent. I was among the early urologists to perform Zenflow. The question I’m asking: Can we relieve obstruction while doing less to the patient?
For one patient, minimizing the procedure and recovery may be the priority. Another may care more about maximizing obstruction relief or choosing the most durable option. Neither priority is wrong. That’s why I’m cautious when a new technology is called “better.” Better at what?
The right BPH treatment depends on the anatomy, severity of the problem, evidence behind the treatment, and what the patient considers a successful outcome. More options are a good thing. The challenge is knowing which option makes the most sense for which patient.

Excited to be just the third site in the country to offer Zenflow, a soft implantable prostatic spring that can be done in office under minimal sedation using a flexible cystoscope - the ideal setting for a true MIST. First cases went off without a hitch!

What an inspiring weekend as an instructor for the “Moses with the masters” HoLEP course this past weekend. The takeaways were clear:
✅ HoLEP really is as amazing in real life as it is on paper
🙋♂️Patients are increasingly requesting it and more and more urologists are rising to the call to offer it
💪🏻It doesn’t take a fellowship to learn, only determination and persistence
