Post REVIVED BCIS-2 period: does CMR play a role in the decision made by heart teams for revascularization ? It was evaluated by us. In this study. Unsurprisingly, we still incline towards CMR for decision which helps in greater agreement in therapeutic decisions. We often use overlapping stents from different platforms in clinical practice. We were interested in understanding what actually happens at the interface between such heterogeneous stents compared with homogeneous stent overlaps. In our retrospective, two-centre study, we found that there was no major difference between heterogeneous and homogeneous stent overlaps in terms of the outcomes we evaluated. @SRIHER_Official @SRMC_official @drsalmans @aiims_newdelhi doi.org/10.1002/ccd.70781

This article by @AndrewFoy82 is a must read for anyone in medicine. External validity of clinical trials may be one of the most under-appreciated concepts in medicine especially when it comes to heart failure therapy. open.substack.com/pub/sensib…

The ideal puncture site can be identified only by echo!!!!! I think that transesophageal or intracardiac #echofirst guidance should become the only way to perform transeptal puncture!
Where is the ideal site for transseptal puncture? In the 40° RAO projection, a pigtail catheter placed in the non-coronary aortic cusp helps define the posterior border of the aortic root. The optimal puncture site (X) is typically located 1–3 cm inferior to the posterior boundary of the aortic root, within the fossa ovalis region. Accurate localization is critical to safely access the left atrium while minimizing the risk of aortic puncture and other complications. Reference: Croft CH, Lipscomb K. Modified technique of transseptal left heart catheterization. J Am Coll Cardiol. 1985;5(4):904–910.

A p-value below the noninferiority threshold is not a free pass. Noninferiority margins need clinical justification, and intention-to-treat and per-protocol analyses should ideally point in the same direction. As DES outcomes continue to improve, noninferiority trials of coronary devices become increasingly fragile.
