Could DESTINY-Breast05 help address one of the biggest remaining challenges in HER2+ breast cancer? Despite major advances in treatment, CNS metastases remain an important unmet need in HER2+ breast cancer. Dr. Mothaffar Rimawi (@mfrimawi) reflects on the early CNS findings from DESTINY-Breast05, where the results appear numerically encouraging. He discusses the possibility that T-DXd could help reduce the risk of brain metastases, while emphasizing the need for longer follow-up before drawing conclusions. What are your thoughts on the early CNS findings from DESTINY-Breast05? Learn more here: communityhealth.media/series…

Can complete ctDNA clearance in SERENA-6 translate into longer survival for patients with HR+/HER2- advanced breast cancer? Drs. Mabel Mardones (@mabelonc) and Anne O'Dea (@AnneODeaMD) point out that 50% of patients in SERENA-6 who switched early to the oral SERD camizestrant at the emergence of an ESR1 mutation achieved complete ctDNA clearance by 8 weeks, compared with just 2% of those who continued on their aromatase inhibitor. While phase 3 survival data are still maturing, both experts discuss why this level of molecular eradication is likely to correlate with overall survival. Does seeing 50% complete ctDNA clearance make a difference in how you view early switching? Share your thoughts below. Learn more here: communityhealth.media/series…

I hate watching experimental therapeutics sessions where they say their drug is active in a wide range of “solid tumors” yet breast was excluded. We need new drugs too! Especially in #TNBC and Luminal B #BreastCancer . Please give us some non-TOPO-1 ADCs!!! #ESMO26

🧠 CNS activity of a TROP2 ADC in a randomized phase 3. #ESMO26 LBA96: brain mets analysis of TROPION-Breast02, Dato-DXd vs chemo in 1L TNBC patients who are not IO candidates. 🧵 #bcsm 61 of 644 patients had asymptomatic, stable brain mets (30 untreated, 31 previously treated). 24 had measurable intracranial disease. 🙌🏻 Intracranial response: ORR 62.5% vs 37.5%, including a complete response 12-week intracranial disease control 75% vs 50% 🙌🏻 CNS PFS 9.5 vs 2.6 mo, HR 0.27 (95% CI 0.13–0.56). Systemic PFS in the brain mets group: 7.7 vs 3.5 mo, HR 0.37. The signal held in *untreated* brain mets: PFS HR 0.29, OS HR 0.35. That's the group where systemic options matter most. Bottom line: Dato-DXd is a reasonable systemic choice for 1L TNBC with brain mets.

A historical HER2-negative result may not tell the whole story. Dr. Sunil Badve highlights the importance of revisiting prior tissue samples or obtaining a new biopsy when appropriate to identify HER2-low and HER2-ultralow expression, helping ensure eligible patients are not overlooked for treatment with T-DXd. Are you routinely reassessing HER2 status in your patients with metastatic breast cancer? 👇

Pathologists are some of my favorite and most intelligent colleagues, and Dr. Badve is a pioneer in the field- I always listen to what he has to say!
HER2-low and HER2-ultralow breast cancers, traditionally classified as HER2-negative, now have a targeted option in trastuzumab deruxtecan (T-DXd). Dr. Neil Iyengar (@Neil_Iyengar) and Dr. Sunil Badve explain why this shift is so remarkable, and why accurate HER2 testing and interpretation matter more than ever. How has this changed the way you test and treat?

When is the right time to introduce T-DXd in HR+/HER2-low and HER2-ultralow metastatic breast cancer? Dr. @Neil_Iyengar reflects on the DESTINY-Breast06 findings, which demonstrated improved progression-free survival with T-DXd compared with conventional chemotherapy, and what these results mean for bringing T-DXd earlier into the treatment sequence. What are you using after endocrine-based therapy in your patients with HER2-low or HER2-ultralow disease? 👇

We now have prospective data with TOPO1 ADC sequencing from: - SATEEN (#ESMOBreast26) - HER3-DXd (#ESMOBreast26) - SWITCH (upcoming #ESMO26) All validate prior retrospective data, showing that TOPO1 ADC resistance is not overcome by target switch. New payloads urgently needed.
#ESMO26 | SWITCH – Phase II In mBC progressing after ADC therapy, switching the target antigen while continuing with the same TOP1 inhibitor payload class resulted in ORRs of only 0–8.3%. Although the sample size was small and the patient population heterogeneous, changing the target antigen alone does not appear sufficient to overcome resistance.

How much HER2 expression is enough to make a difference? The DESTINY-Breast04 and DESTINY-Breast06 trials have changed how we think about HER2 expression in metastatic breast cancer, expanding treatment possibilities for patients with HER2-low and HER2-ultralow disease. Dr. Sunil Badve uses an analogy of moss, grass, shrubs, and trees to explain how even very low levels of HER2 expression, once considered clinically insignificant, may still be relevant when identifying patients who could benefit from T-DXd. How are you approaching treatment decisions for patients with HER2-low and HER2-ultralow disease? 👇

How long should patients stay on T-DXd + pertuzumab? In DESTINY-Breast09, the median time to complete response was approximately 8 months, while the median time to maximum tumor reduction was around 11 months. Dr. Irene Kang (@irenekangmd) discusses why these findings have changed how she thinks about treatment duration in first-line HER2+ metastatic breast cancer, particularly the importance of giving patients enough time to achieve their best response rather than transitioning to maintenance after just 6–8 cycles. How are you approaching treatment duration with T-DXd + pertuzumab in your practice?

ESR1 mutations are more than a marker of endocrine resistance. They're actionable. Drs. Jason Mouabbi (@JAMouabbi) and Erika Hamilton (@ErikaHamilton9) discuss the evolving treatment landscape for HR+/HER2− metastatic breast cancer, including the importance of repeat ESR1 testing and how different mechanisms of action may influence treatment decisions. In the longer discussion, they take a closer look at vepdegestrant, how its PROTAC mechanism of action differs from oral SERDs, and how emerging insights into endocrine resistance may help shape more individualized treatment approaches. How Are You Using Liquid Biopsy to Detect ESR1 Mutations? When testing for ESR1 mutations, having a broader picture of the disease can help inform treatment decisions. Dr. Erika Hamilton (@ErikaHamilton9) explains how liquid biopsy (ctDNA testing) can complement tissue NGS by detecting ESR1 mutations shed from different metastatic sites, offering additional insight into tumor biology when selecting subsequent endocrine therapy. How are you approaching ESR1 mutation testing in your patients?

T-DXd Expands Treatment Options for HER2-Ultralow Breast Cancer With T-DXd now an option for eligible patients with HER2-ultralow metastatic breast cancer, even very low levels of HER2 expression can make a difference in treatment selection. Having another treatment option is a meaningful step forward for patients who previously had fewer choices. What difference has T-DXd made for your patients with HER2-ultralow disease?
