78M, fatigue and an unwitnessed fall, now nauseous. What's the level of block, and what would you check before calling it? A) It's nodal; the QRS is narrow. B) It's infranodal; the PR is fixed. C) It's complete heart block. D) Can't tell yet; I'd want a longer strip. #EPeeps #ECG

What I'd check: a longer strip for a 3:2 run (PR lengthening means nodal) or two conducted beats on a fixed PR before a drop (infranodal). Long PR with a narrow QRS leans nodal; normal PR with bundle branch block leans infranodal. This one is neither.

Heart rate helps too. Exercise or atropine speeds AV nodal conduction, so nodal block tends to improve. It also speeds the sinus rate, so diseased His-Purkinje tissue falls behind (3:1). Vagal manoeuvres do the reverse. Still unclear? An EP study can measure the HV interval.

A 3D activation map colours each point by the time the system annotated, so an early zone is only as reliable as the signal that was annotated. In this focal AT, the RA site the map called earliest had its local signal inside the P wave and an rS unipolar; the earliest deflectio

One of the great lies of EP training is that exposure automatically becomes understanding. It doesn't. Watching 500 cases doesn't magically teach someone why those cases behaved the way they did. #EPeeps #CardioEd

Stochastic time on a simulator isn't learning. Neither is stochastic time in the lab. Plenty have done 500 posterior wall isolations but understand little about septopulmonary bundle anatomy, LA intramural fibre architecture, and how they drive posterior wall recurrence.

Time served is not the same as things learnt. Not in a simulator. Not in a conference. Not in an EP lab. There are no shortcuts in EP. The sooner you accept that, the sooner you'll "shortcut" to understanding. cardiacphysinpractice.com/ep…

All those years of me saying…I wouldnt use PFA on the CTI…perhaps nonsense. RF may be no better. In the IMPACT-CTI what mattered wasn't PFA versus RF. It was the patient's coronaries. #EPeeps #EPfellows

None reached a significant stenosis, and PFA patients got IV GTN first. Retrospective, not powered to call the energies equal BUT very thought provoking. I wont defend RF anymore. Should we check coronary history before ablation?

Like it or not, 3D mapping techniques have become intertwined with traditional EP concepts and EGM interpretation, so much so that no EP course is complete without it. #epeeps #epfellows #electrophysiology #egms #ecg #ekg #cardioed #meded #foamed

Raising the standard in EP training starts with a robust systematic method, not on the job chance exposure followed by a vibes based sign-off after enough supernumerary time is served. It's not like this everywhere, to be fair…but in far too many places it is….grrr…. #EPeeps
