Hospitals need constant communication with #caregivers BEFORE they reach crisis status at home.
Crisis at home greatly affects the health of the patient - their outlook, anxiety levels, recovery times and likelihood of re-admittance into the hospital.

Each person's diagnosis is unique.
That means each #caregiver's journey is also unique.
I wish I could give absolute answers, the reality is that no one can.
