🎯 「肺静脈隔離（PVI）のためのpulsed field ablation（PFA）は、意図せずVein of Marshall（VoM）にも影響を及ぼしているかもしれない」という論文です。
今回のゲストは 中谷 洋介 先生（群馬大学 @yosuke3gbst）🇯🇵
PFAによるPVIを受けた72例を対象とした前向き観察研究。FARAPULSE、VARIPULSE、PulseSelectの3つのPFA systemを用い、VoM内にmicroelectrode catheterを留置して、PFA中のVoM電位をリアルタイムに評価しました。
注目は、PFA直後には44%でVoM電位が変化したものの、手技終了時まで変化が残ったのは14%だったこと。そしてVoMへの影響を左右したのは出力エネルギーの強さ以上にPFA electrodeとVoMとの「距離」でした。

We have known about #WPW and accessory pathways as a common cause of SVT for >100 years and have effective procedures to address them, but we still have NO IDEA why some patients are born with these extra slivers of muscle that electrically connect atria to ventricles. Medicinal use of lithium in pregnant women leads to heightened risk of a particular form of congenital heart disease associated with these pathways. Might variable levels of lithium in drinking water affect the chance a growing fetus will be born with such a pathway? @jclinicalinvest @UCSF @UCSFHospitals @stopafib @DrDave01 @KnowYourPulse
