Congratulations to Prof. Michael Shapiro @DrMichaelShapir on receiving the Arthur Agatston Award @Agatston_Center at #SCCT2026, a well-deserved recognition for his contributions to preventive cardiology and cardiovascular imaging #YesCCT. His lecture, “How Looking at Plaque Made Me a Preventive Cardiologist,” traced preventive cardiology’s evolution from population-based risk estimates to directly visualizing atherosclerosis and personalizing risk. The question has shifted from “How likely is disease?” to “How much disease is already present?” and CAC scoring has been central to that shift. One line that stuck with me: “Don’t guess, when you can know.” This is also repfected in the 2026 ACC/AHA Dyslipidemia Guidelines, where CAC scoring now carries a Class I recommendation to guide lipid-lowering therapy decisions when uncertainty remains. Looking ahead: earlier detection with CCTA, AI-powered plaque quantification, opportunistic CAC, and imaging-biology integration for truly personalized prevention. A fantastic reminder that the goal isn’t just predicting the first heart attack…it’s preventing it. @Heart_SCCT @ChinnaiyanMD @AnamWaheed05 @SiddharthT22 @RooshaParikh @AdyOrbach @kfarooqi @Sot_Evangelou

Great Ron, we 'got' this in the @NHSEngland in our language, we are in the process of getting this for all in the UK. @DrDerekConnolly @HarrisSital @DrScottMurray @Paddy_Barrett

Regarding the VA AI calcium score study (ai.nejm.org/doi/full/10.1056…): Our team at @CVRF_SoCal led one of the first FDA-cleared validation of AI coronary calcium scoring but not from chest CTs (pubmed.ncbi.nlm.nih.gov/3713…). In 100 non-contrast CACs, AI matched 3 expert readers with R=0.996. Using CAC-DRS calcification system, AI showed 86% agreement; 14% were reclassified—mostly due to underestimating RCA calcium or overestimating RV density for CAC 0–1 which can be clinically relevant to treatment. Overall accuracy of our study: 89%, aligning with the VA study of 84% using chest CT - encouraging for wide-spread adoption. @khurramn1 @CotsCardiology #calciumscore @Heart_SCCT @DrMarthaGulati @EricTopol @RonBlankstein @CVMGBevHills @CAIALOfficial @BudoffMd #artificalintelligence @elonmusk @WHO @BhavikPatelMD @ACCinTouch

Khurram we are planning this in the UK we have @CorelineSoft lined up. 1.4 million scans, let's see what we can do @BirkhoelzerS @DrDerekConnolly @HarrisSital @DrScottMurray @RonKarlsbergMD @TheBHF @BritishCardioSo @DailyMail
AI-CAC enables coronary calcium scoring from routine chest CTs 20K scans, 98 VA sites, 89% accuracy From my perspective, time to connect AI outputs to action, to truly maximize implementation-->prevention ai.nejm.org/doi/full/10.1056… #DigitalHealth @HoustonMethodist @RiceUniversity

We can do this for coronary disease @HarrisSital @RonKarlsbergMD @DrDerekConnolly @DrScottMurray @DHSCmedia, were here to help, and willing @MyHeartPrevent trials "oven ready" @DrRiyazPatel @mdstbarts
The revolution in medical science and technology is one of the greatest opportunities for humanity and we’re putting the NHS at the forefront of it. We’ll not only be able to diagnose and treat illness more effectively, we’ll be able to predict and prevent it.

We can do it for common diseases as well, coronary artery disease for starters @TheBHF @BritishCardioSo, I think there's a webinar tomorrow night??
Replying to @David_Cameron
But get this right & it could be transformational for preventative & personalised healthcare. I know – as a father of a child with a rare disease & now through my work at @OHRareDisease – what this could mean. Well done @wesstreeting, keep going!
