The nursing process is not an optional extra. It’s fundamental to nursing. From day 1 we learn to assess, diagnose, plan, implement & evaluate. With every placement, experience and reflection, we build on that. It’s clinical reasoning in action.

Rather than rushing to judge individuals on the basis of a report in a newspaper because it reinforces your bias against advanced practice, it might be worth digging in further to find out about the systems and the specialities involved before apportioning blame.

History, examination, anatomical localisation and differential diagnosis are fundamental. But clinical errors aren’t exclusive to one profession, nor are good clinical decisions exclusive to doctors.

I’ve identified serious illness missed by senior doctors and escalated cases needing specialist intervention. Judge competence individually, learn from errors, and examine the systems around them. Patient safety isn’t a professional turf war.

As a Dr when teaching med students I warn them that EVERY single near miss or avoidable death I’ve seen in >30y has been where 1)failed to take a Hx 2)failed to examine patient 3)failed to localise anatomically 4)failed to come up with differential diagnosis. This👇is 1)-4)🤬
Dylan Cope died after an A&E nurse practitioner mistook his abdominal pain for flu, having ignored his GP’s referral for suspected appendicitis. His family thought she was a doctor. A coroner found he would have survived had a senior doctor examined him. telegraph.co.uk/news/2026/10…

Dylan’s death is heartbreaking. His family deserve compassion, answers & meaningful action. The failings must be examined, inc individual decisions & wider org systems. But one tragic case, does not prove an entire prof group is unsafe. We need to learn from the failures.

The @NHS should not need to have volunteer roles. IMO!

Happy to debate with anyone who can manage a sensible, evidence-based discussion but the pile-ons, digs & deliberate attempts to stir up trouble are beneath contempt. #ReportAndBlock

How’s your afternoon going? Lots of nastiness over the weekend. One was actually severe enough to report to the Police. #ReportandBlock #advancedpractice

This comprehensive thread by Neena still does not prove that another healthcare professional cannot learn this very specific thing, including red flags and differentials etc.
“Why can’t a paramedic just do training in breast lumps & then see breast patients like a GP?” Thank you for asking this. This question underpins the lack of understanding of medical training & the very foundation of medicine. I’ll explain why this is so unsafe … 🧵 1/n

In my specific ‘breast lump’ example, it easily create a 100+ node clinical reasoning tree from one complaint. That is comparative to a shortness of breath example faced by Para’s everyday - don’t tell me we couldn’t train a paramedic to do this.

1, Lecturers in #AdvancedPractice Do you teach that AP is not about replacing Drs but about developing AP within their core profession? & in your experience do students generally understand and embrace that distinction…

2, or do some enter advanced practice with the perception that the goal is essentially to become a replacement for another profession? Genuinely interested in how this is approached across programmes. @thehaighs @alisonleary1 @AmandaHensman @caspertown24
