Humbled to receive the Nevada ACP Governor’s Chapter Service Award!
Truly didn’t expect this. My focus has always been simple: supporting colleagues, driving research, and expanding medical education.
Grateful to @ACPIMPhysicians Nevada leadership and my peers. Onward! 🩺❤️

Need a small RCT with a digoxin strategy for acute HFrEF. It’s sometimes tough to disentangle the effects of acute digoxin loading from vasodilator titration, but anecdotally there seems to be a benefit - particularly in new onset HFrEF in “shocky” patients and chronic patients not candidates for advanced therapies (oftentimes coupled with deprescribing beta blockers)
@maz_hanna
In a series of 10 low-output HFrEF patients, IV digoxin loading and vasodilator titration were associated with increased median cardiac index from 1.45 to 2.70 L/min/m² at 24 hours; all four on IV inotropes were weaned off. Time for an RCT with digoxin in low output HF.

🔥Outstanding analysis and perspective by Dr. Mandeep Mehra in JACC HF on AHTC Fellowship training in US
🔥’AHFTC crisis is not a referendum on the intellectual appeal or clinical importance of HF medicine; it is a verdict on the design of fellowship and early-career proposition’
🔥Many parallels outside AHFTC as well
🔥Early leadership concept will be challenging since it needs to consider those mid career future leaders-in-waiting in the leadership pipeline

Does the flu shot protect more than your lungs?
New episode of The Heart of Prevention: Dr. Keerthana Pakanati @Kpakanati3 sits down with Dr. Alison Bailey @a_l_bailey to unpack the growing link between influenza and cardiovascular disease, including what the cardiovascular analysis of FLUNITY-HD reveals about heart failure hospitalization risk.
They dig into where flu vaccination might fit alongside statins and antihypertensives in the preventive cardiology toolkit, how to think about high-dose vs. standard-dose vaccination in older adults, and what evidence gaps remain.

In patients with #AFib and intermediate stroke risk, direct oral anticoagulation led to a lower risk of stroke, systemic embolism, major bleeding, or death from cardiovascular causes than no anticoagulation. Full SINGLE-AF trial results and Research Summary: nej.md/4ii61XB

ALT This image contains an excerpt from the Research Summary "Anticoagulation for Atrial Fibrillation with Intermediate Stroke Risk" published in the New England Journal of Medicine. It includes a visual representation of a patient with atrial fibrillation and an intermediate risk of stroke. Patient demographics are detailed as 1803 adults, average age of 60 years. The bottom half contains illustrations of the two treatment options: direct oral anticoagulant (DOAC) therapy or no anticoagulation. ✅️MRAとSGLT2阻害薬がLVEF問わずClass I(HFpEFでは非ステロイド性MRAも可‼️)
✅️GDMTをFMT(基盤治療)とAMT(追加治療)に再定義、デバイス/カテーテル治療はGDIT
✅️肥満合併HFにセマグルチド/チルゼパチド(IIa)が初登場

Reminder 🩺 New CardioCore Lecture: Hypercortisolism — Recognizing a Hidden Driver of Cardiovascular Risk
Wednesday, August 26, 2026 | 6:00–7:00 PM EDT
Some patients keep facing hypertension, diabetes, obesity, and accelerated CV disease despite optimal risk factor management. Hypercortisolism may be an underrecognized, treatable cause.
Join Dr. Jorge Plutzky @jplutzkymd (Director, Preventive Cardiology, Brigham and Women's Hospital) as he covers the pathophysiology, clinical features, and cardiovascular consequences of hypercortisolism — plus when to suspect it, how to screen, and referral pathways. Moderated by Dr. Charles German @DrGermanMD (Georgetown University).

Simultaneous/rapid sequencing to all four components of quadruple GDMT at 🎯 doses (as well tolerated) should be the standard of care for all
