GI doctors get colonoscopies too!
Dr. @hgagneja, Vice-Chair of the ACG Board of Governors, walks you through the process with his recent colonoscopy.
If you're 45 or older and at average risk, talk to your doctor about colorectal cancer screening.
Prevention is powerful.🩵

Nearly half of new colorectal cancer cases now occur in adults under 65.
The epidemiologic shift is real and it's not subtle.
Organized screening drove incidence down in the 65+ population. That was the success story. But the age-incidence curve is now moving in the wrong direction for younger cohorts.
The generational gradient is striking. Someone born in 1990 carries roughly twice the colon cancer risk and four times the rectal cancer risk of someone born in 1950.
What I keep coming back to clinically is stage distribution. Three out of four patients under 50 present with stage III or IV disease. We're seeing these patients after nodal involvement, after peritoneal seeding, after liver mets — not because the patient ignored symptoms, but because their providers did. Iron deficiency anemia in a 38-year-old gets worked up differently than in a 68-year-old. It shouldn't.
I see this in clinic every week. The patients getting diagnosed at 40 with synchronous liver metastases who were symptomatic for months before anyone scoped them.
The biology of early-onset CRC may also be different — higher rates of signet ring histology, left-sided predominance, distinct molecular profiles. We're still learning what that means therapeutically.
The demographics of this disease have already changed. The question is whether our diagnostic reflexes are keeping pace.

"Immediately, I was Rip Van Winkled. I fell into an oceanic unconsciousness — did a high dive into eternity, circled the solar system infinite times, breathed the breath of my ancestors, became everything and nothing."
This piece deserves a Pulitzer.

A 42-country analysis of change in cancer patterns with age indicates only colon cancer is clearly on the rise in the young (age 20-49 years). Most of the rest of 12 cancer types were increasing across the age groups.

I was in academics for 20 years, and was a full Professor of Medicine with Tenure.
5 years ago I left for a large private GI group.
People often ask me about pros and cons of both worlds.
Here are my top tips to consider.

I highly encourage any GI provider interested in the business side of their practice to join me and @kunjali for this year’s @AmCollegeGastro practice management summit on Friday Oct 24, 2025!
The topics are incredibly practical: from succession planning, new ancillaries (anorectal/CCM), masterclass on AI in the clinic, optimizing your ASC utilization to lessons learned from private equity.
These are suitable to academic, hospital-based or private practitioners.
For me this is the true hidden curriculum of this field.
