In patients with chronic coronary syndrome taking oral anticoagulants, adding aspirin led to higher risks of cardiovascular events, major bleeding, and death from any cause than anticoagulation alone. Full AQUATIC trial: nej.md/45AU4Wp

A guideline-driven VTE prevention program (Vermont Model) led to ~60% of high-risk cancer patients receiving prophylactic anticoagulation—resulting in a 60% reduction in VTE and 56% reduction in death. #ISTH2025 #VTEprevention 🩸📉

Do you dose reduce DOAC for extended prevention of VTE?
💙 Post hoc analysis of RENOVE trial of secondary prevention of VTE.
♥️ suggests low dose DOAC may be inferior to full dose among people with BMI >30.

🩸 New insights from the RENOVE trial (extended duration anticoagulation) at #ISTH2025:
📉 Reduced-dose DOAC is non-inferior in patients with BMI <30, with less bleeding risk.
⚠️ But in patients with BMI ≥30, reduced-dose may be inferior for bleeding and recurrent VTE.

RENOVE trial: Full dose vs low dose extended anticoagulation in pts requiring long term AC.
Conclusions:
-similar low rate of recurrence (~2%) in both groups
-lower rate of bleeding w low dose
-a chance to compare DOAC efficacy (same!)
-caution w obesity

@ASH_hematology Late Breaking Abstract:
Should patients receive extended duration anticoagulation with reduced or full dose DOAC for the secondary prevention of recurrent VTE - The RENOVE RCT presented by @FCouturaud
Reduced dose DOAC was not non inferior to full dose ((5-year cumulative incidence 2.2% vs 1.8%, P=0.23 for noninferiority) but lower risk of clinically relevant bleeding (9.9 vs 15.2%)! 🩸
Risk benefit seems to favor reduced dosing for most patients #ASH24

Patients with MPN and splanchnic vein thrombosis who are not receiving anticoagulant therapy seem to have a higher risk of recurrent VTE, bleeding complications and mortality compared to those receiving anticoagulation at 12 months.
