Agree, of we’re not all moving to Bluesky or Roon, we might as well ask me on here and intermittently clean our feeds to keep it all the politics/racism #IDtwitter forever!
An unpopular opinion: The Bluesky experiment for the ID community has officially failed, IMHO. Remember when so many ID folks migrated? Did we build a stronger community—or just split it across platforms? #IDWeek2026 #IDXposts @BradSpellberg @PaulSaxMD @PulmCrit @IDstewardship

A 6m course of Terbinafine, including drug monitoring: ~£50. 3d inpatient stay for cellulitis (in UK): ~£2700 This gives a NNP (Number Needed to Prevent) of ~54. Maybe you don’t want to Rx everyone with onychomycosis; but if you’ve already had cellulitis? Worth it I think
We shouldn’t be doing fungal nails on the NHS. Fight me.

Top level procrastination. Well done @PaulSaxMD Our first ‘series’ on bacteria was planned to last 2 years total, that deadline whooshed by 3 years ago.
PowerPoint deadline season in academic medicine, w/ an inbox overflowing with "friendly reminders" to submit slides and other stuff weeks before my talks! Naturally, instead of finishing those slides, I wrote this to complain about it. Free link in replies. @NEJM

I answered this, but I answered that I’d definitely oral switch for every eventuality so I think I’ve skewed your poll a bit. Sorry @DrToddLee
Looking for people who treat S aureus bactermia to weigh in on some questions about oral switch and what type of data is required to permanently move the needle. Please answer honestly if you treat these patients shorturl.at/ShTWQ @ABsteward pls amplify :)@DrEmilyMcD

‘Fun’ fact (it’s not that fun): they did. Treponemal disease is native to the Americas, that’s why the non-syphilis treponemes are all in South America. Syphilis was brought back into Europe and then the rest of the old world as a result of -ahem- *contact* with American peoples
you know how native americans were utterly anhiliated by european diseases they have no immune response to? how come the europeans didnt catch native american diseases in the same way

PROPOSAL. OutPatient Antibiotic Therapy. No COPAT. no ‘Parenteral’. Only OPAT. No difference between oral and their inferior IV counterparts. That is all.
Replying to @apsmunro
Oof how was that received? I don't even remember what OPAT stands for lol

Gotta share this: In the MDT we’re discussing ABx therapy in a 93yo, enterococcus IE. Final recommendation: “Start on the therapy most associated with good outcomes (Linezolid in this case): when side effects limit use, StepDOWN to the less effective IV option(Dalbavancin)”

Stop using step down! 📢📢

Not often that #IDXposts traces me something I had no idea about but this was really a revelation: Quinolone cross-reactivity traditionally low. Thanks @KASIC_MDRO ! Still hate quinolones though
A 44-year-old female is on day 3 pip-tazo for a complicated UTI. Her urine culture grows P. aeruginosa and she is ready to go home. She has a history of rash to ciprofloxacin. Can she be discharged on levofloxacin?

Wish Empiric the card game and Sanford the antibiotic guide had a baby? Well they finally did! Check out the new & improved Empiric! Easy to learn & fast to play. I actually really like how you can create your own mini-Sanford by fanning out individual cards. ☺️
