"Durability of pulmonary vein isolation: Does the pulsed field ablation system matter?" @PrashSanders et al in @hrs_journal Oct 2026 @HRSonline
✴️An interesting challenge to our assumptions.
➡️ Question - Are PFA systems comparable for
long-term PVI durability after equivalent acute PVI?.
➡️Let's start with a poll for #EPEEPS
✴️ Which PFA platform do you currently favor for AF ablation?

🔥 Hot off the press: We identified how and where AF begins.
I am excited to share this work.
Since the discovery of PV triggers more than 25 years ago, AF ablation has evolved into an empiric therapy centered on PVI. Yet, we have never been able to identify the arrhythmogenic substrate that determines why a premature beat initiates AF at one site and not another.
We believe this study changes that.
In this mechanistic study, we found that AF initiation arose predominantly through reentry originating from a small number of spatially discrete regions with steep repolarization gradients. Their distribution was unique to each patient, and they accounted for more than 92% of AF initiation episodes.
Importantly, the arrhythmogenic substrate was not defined by low voltage, and while conduction abnormalities were often present, they appeared to play a secondary role.
Its patient-specific distribution may help explain the controversy surrounding posterior wall isolation: the posterior wall harbored the substrate in only approximately 30% of patients. Importantly, in redo patients with durable PVI, right-atrial substrate (which is often beyond standard mapping strategies) was identified in 30%.
Most encouragingly, in an exploratory cohort of 24 patients with recurrent AF and durable PVI (50% PerAF), targeted elimination of these sites achieved 87.5% single-procedure freedom from any atrial tachyarrhythmia over a median of 489 days, without antiarrhythmic drugs.
The thoughtful accompanying editorial by Jon Kalman highlights the significance of this discovery and its central translational challenge of bringing it into real-world clinical practice.
But stay tuned! Technology is under development to transform this discovery into a rapid, automated functional mapping.
Current electroanatomic mapping relies largely on voltage amplitude and activation timing. We need to move beyond these measures and map how tissue actually functions - how it conducts, repolarizes, and responds to premature stimulation. This can add the critical dimension that has long been missing from substrate mapping and may reveal what truly makes cardiac tissue arrhythmogenic.
To me, this is the beauty of physiology: there are no black boxes. We can identify the sites that reproducibly initiate the arrhythmia, define their EP properties, eliminate them with ablation, and confirm that arrhythmia can no longer be initiated.
The next steps are prospective multicenter validation and technological development. Together, they may move AF ablation beyond empiric therapy toward truly individualized, mechanism-guided therapy.

🏃♂️⚡ Can exercise stress testing (EST) improve the diagnosis and risk stratification of #BrugadaSyndrome?
A fascinating new state-of-the-art review highlights that exercise testing is much more than a tool for assessing exercise capacity. In Brugada syndrome, the *recovery phase* may be the key moment to uncover concealed disease and identify patients at higher arrhythmic risk.
🔍 Key takeaways:
✅ Type 1 Brugada ECG pattern is rarely unmasked during peak exercise (~1%) but becomes evident in up to 25% of patients during early recovery.
✅ The transition from sympathetic activation to parasympathetic rebound appears to play a crucial role in ECG changes and arrhythmogenesis.
✅ Premature ventricular contractions (PVCs) occurring during early recovery may represent an important marker of future ventricular fibrillation risk.
✅ Exercise-induced malignant arrhythmias are rare, suggesting that EST is generally safe when performed with appropriate monitoring.
✅ Careful ECG monitoring during the first minutes of recovery may provide valuable diagnostic and prognostic information.
💡 The review supports the concept that both *peak exercise* and *early recovery* offer complementary insights into the electrophysiological substrate of Brugada syndrome and may help refine individualized risk assessment.
An important reminder that sometimes the most informative part of a stress test is not the exercise itself—but what happens immediately afterwards.

🔍 #EHRATopicWeek on Atrial Tachycardias
Atrial Tachycardia is more complex than we once thought
Traditionally, rapid regular atrial arrhythmias were divided into “atrial tachycardia” and “atrial flutter” based mainly on ECG appearance. Today, high-resolution mapping shows these distinctions are often oversimplified.
📌 Current concepts classify all regular atrial rhythms >100 bpm as AT:
• Non-reentrant AT
• Reentrant AT (localized or macro-reentrant)
Importantly, many arrhythmias previously labeled as “flutter” may actually represent different AT mechanisms.
⚡ Differentiating AT from organized AF can be challenging, especially in scarred atria or after ablation. ECGs, EP studies, intracardiac mapping, and even wearable devices all play an important role in diagnosis.
💡 Precise classification matters — for diagnosis, treatment strategy, and improving ablation outcomes.

More than 10 years ago Oussama and I started our journey of investigation, trying to the define the optimal first treatment for patients with atrial fibrillation.
Separately we pursed randomised studies in populations with treatment-naive paroxysmal AF.
Five years ago we presented our RCTs in the paroxysmal AF population, which ended up sitting side by side in the same issue of the New England Journal of Medicine.
nejm.org/doi/full/10.1056/NE…
nejm.org/doi/full/10.1056/NE…
Since then we have been working together to examine the same question of optimal initial therapy, but instead looking at patients with more advanced forms of atrial fibrillation.
Today Oussama presented the results of the AVANT-GUARD study, which definitively demonstrated the value of initial catheter ablation in the treatment naive persistent AF population.
Like EARLY-AF, AVAT GUARD used implantable cardiac monitors to evaluate arrhythmia endpoints
Like EARLY-AF - initial catheter ablation reduced recurrences in AVANT GUARD by about 50%, which was about 25% in absolute terms. To put this another way, only 4 patients need to be treated with ablation to prevent a recurrence of arrhythmia.
However, binary recurrence doesn’t tell the whole story. Because we had loop recorders we were able to assess the time spent in AF (or AF burden). In this regard significantly more patients had none or negligible burden after ablation (<0.1%), with significantly more AAD treated patients having a very high burden (>5%). This is important as AF burden is associated with clinical outcomes such as hospitalisation.
In terms of risk - the rates of any adverse event or serious adverse event were comparable between randomised arms. Highlighting that persistent AF patients with high CHADS-VASc score are high risk irrespective of treatment provided. This is important as we reflect on treatment options as part of shared decision-making.
Congratulations to Oussama for the presentation and leadership, to the co-investigators and study sites for their participation and dedication, and a huge thank you to the Boston Scientific team for their support and commitment to advancing science.
To end, I want to acknowledge and thank Oussama for this shared journey, I’m grateful for our collaboration and friendship, and look forward to what we will do next.
Presented at #HRS2026:
In patients with persistent atrial fibrillation, first-line treatment with pulsed field ablation reduced the recurrence of atrial arrhythmias at 12 months as compared with antiarrhythmic drugs. Full AVANT GUARD trial results: nejm.org/doi/full/10.1056/NE…
