🚨Excited to share our PELICAN Randomized Clinical Trial, just published in JAMA (D1, IF 65.4) 📄 Individualized Cardiac Index-Guided Hemodynamic Therapy in High-Risk Patients Undergoing Elective Major Abdominal Surgery: The PELICAN Randomized Clinical Trial 🌍 A large, international, multicenter RCT conducted across 12 university hospitals in Austria, the Czech Republic, Denmark, Germany, and Spain. 🔬 The clinical question: Does individualized intraoperative hemodynamic therapy targeting each patient’s preoperative resting cardiac index improve postoperative outcomes compared with routine hemodynamic management? 👥 1,095 patients included in the primary analysis. 📊 Key findings: Major postoperative complications occurred in 44.3% of patients receiving individualized cardiac index-guided therapy versus 46.9% receiving routine care. Relative risk: 0.94 (95.1% CI, 0.83–1.08; P=.38). No significant differences were observed in acute kidney injury, myocardial injury, severe infectious complications, cardiac arrest, or mortality within 7 postoperative days. 💡 Clinical implications: Despite the physiological rationale for individualized hemodynamic optimization, targeting the patient’s preoperative resting cardiac index did not significantly reduce major postoperative complications. These findings challenge the assumption that more individualized hemodynamic targets necessarily translate into better clinical outcomes and provide important evidence to inform perioperative hemodynamic management. 🤝 Grateful to all investigators, participating centers, and collaborators for their outstanding contribution to this international research effort. Publishing this work in JAMA, one of the world’s leading medical journals, represents an important milestone for our collaborative research. acrobat.adobe.com/id/urn:aai… #JAMA #PELICANTrial #Anesthesiology #PerioperativeMedicine #Hemodynamics @sedar_es @SCISEDAR @MontesinosSanti @mjcolomina @DrFDamico @JG_Anestesia @FernandoRamasco @bio_critic @BadenesR @MarinaVarelaDu1 @giovannilandoni @germ_red @ClinicaNavarra @JAMA_current

In @JAMA_current, we report that a single African ancestry-enriched CD36 variant (Y325*) contributes more to the population burden of dilated cardiomyopathy (DCM) in individuals of African ancestry than TTN truncating variants or other known DCM genes. ja.ma/3Wy9k40

Congratulations to the team on the publication of NRG-GU005 in @JAMA_current! 👏 This phase 3 randomized trial provides important new data comparing SBRT vs moderately hypofractionated IMRT for intermediate-risk prostate cancer. Proud to see Dr. Grubb and Dr Marshall among the investigators! 🔗 pubmed.ncbi.nlm.nih.gov/4259… @EMW2000 @MatveyTsivian @JL_Drerup @SES_AUA @UroOnc @SUO_YUO @GenevaMaeP @benvstone

Big day for @Research_Inter as the VASSPR n=3959 cluster-randomized trial comparing early addition (0.1 NE) of vasopressin to later (0.4 NE) in septic shock out in @JAMA_current 🎉 @ipeltan & team! jamanetwork.com/journals/jam… Did not find a difference in 28d mortality, dialysis, LOS, vasopressor free days Maybe more cardiogenic shock (?afterload effect) with vasopressin early strategy.

Extremely excited to have been a part of this incredible work on improving how medical urgency is operationalized in US heart transplantation. Please check out our new paper in @JAMA_current! @gracelyden @Nikhil15 @KiranKhush1 @WF_Parker @StanfordSurgery @OpNotes
A new #HeartTransplant allocation score, the US Candidate Risk Score, demonstrated better discrimination of 6-week mortality among adult US transplant candidates than the current 6-status system. In match-run simulations, reordering candidates by this score prioritized patients with higher observed mortality without transplant while not increasing donor-candidate distance. This finding supports use of more objective medical urgency criteria in US donor heart allocation policy. ja.ma/4jRbeGD
