Long post in which I will sound callous and dismissive to many- apology in advance. There seems to be a lot of discussion and concern with all of the “mental health professionals” and their roles, relationships, rapport, etc. From an insiders perspective- the mental health professionals were basically NPC within a pediatric gender center. The patients came to the clinic already socially transitioned, with NO mental health professional input. They had a first visit to get started on hormones with little to NO mental health professional input. I then helped them get seen with a “community based therapist” usually an LCSW or some similarly trained person well below a psychologist training. It was never a psychiatrist- they didn’t write letters. The LCSW saw the patient a handful of times- often ONCE only and a letter was written. I usually stay rather quiet about this- because so much of the conversation seems to be about or driven by the “mental health professionals”but I have still not figured out why so many don’t realize no one really cared about them. The ones who were driving this train were the prescribers. The MDs writing the damn scripts. Why aren’t we talking to and about the adolescent medicine doctors who prescribe birth control and hormones to everyone with a pulse and the endocrinologists? Why can’t we actually recognize that the therapists not only didn’t do much of anything on the front end, won’t do much on the backend either- just make the script writing for these horrid things illegal.

This week a friend asked me: “If teen girls were caught up in a culture-bound mass psychosis, wishing to have their healthy eyes surgically removed, would we still be debating it? Or would we have stopped it right away?” I think it shows our Puritan roots still haunt us. Human sexuality remains the great taboo. We can remove or damage a young person’s capacity for sexual function, and the people who object are the ones accused of being “fixated on genitals.” Naming the loss is treated as more shameful than causing it.

Why did I blow the whistle? Because these interventions don’t work. Period. How do I know this? Because our patients told us so. Because in the privacy of the doctor’s office, where no therapist is judging, where no teacher who changed their name is listening- they knew. They knew their health was worsening, they were tired, still sad, still lost, their labs were worsening. Sometimes they knew in a matter of months, sometimes years later. But they knew.
