I thank @SAMHQglobal for their interest in our article and supporting our recommendations from Portland.
Pediatric anesthesia morbidity predominantly results from respiratory issues, and a simple system may be helpful in limiting the morbidity.
Gosh, it’s already been 3yrs! – at Portland (Vic.), Victoria
This is tremendously important & relevant: "Setting up a Universal Pediatric Airway Trolley for anesthesia departments in nonpediatric centers bit.ly/38YsR1i " @ASALifeline @APSForg
Always nice to see your article published. Confident this article will interest anyone interested in the history of anaesthesia or #RegionalAnaesthesia.

Published on March 2026 issue of Journal of the Japan Society of Medical History, this article summarizes the historical development of brachial plexus blocks in pre-World War II Japan. Anyone with an interest in the hx of regional anesthesia may find it interesting. #anesthesia

For those who add a2 agonists to their PNBs.
Never been too keen on perineural adjuvants, but it’s interesting that when dexmedetomidine is given IV, it may offer “neuroprotection” to the brain - “central nervous system”. But, can be paradoxically “neurotoxic” to the nerves🤔

Very helpful perioperative discussion points with patients who are breastfeeding.
These discussion points will complement any patient info sheet we may hand out to patients. Any recommended ones?
The Australian Society of Anaesthetists has a good one.

✋Can regional anesthesia preserve finger movement during hand surgery?
Motor-sparing forearm blocks allow intraoperative motor testing while providing surgical anesthesia supporting better functional outcomes.

An interesting study that adds useful perspectives to Propofol vs Sevoflurane debate.
To me, it reinforces the msg that the choice of anaesthesia needs to be personalized to the needs of your patient.
Could the findings be extrapolated outside of cardiac surgery patients?

1. RA + opioid-free > RA + opioids
2. Effective RA >>>>> intravenous analgesia
The meteanalysis confirms impressions most of us probably have instinctively noticed in daily practice.
Concerns about opioid-related adverse effects have increased interest in opioid-free anaesthesia, BUT the benefits compared with opioid-inclusive techniques, especially in the presence of regional anaesthesia, remain uncertain.

This msg is very important.
The magnitude of analgesia derived from any IV analgesia cannot compete with effective RA.
The cornerstone of effective intra- and post-op analgesia = RA
Effective transition from RA-based to non-RA analgesia post-op is the next key phase.
When regional anaesthesia provides effective coverage of the expected pain trajectory with appropriate duration, intra-op opioid use may offer limited additional benefits for pain control.
Regional techniques also minimised postoperative opioid requirements.

Ingeniously attempted. The plot thickens with anaphylaxis during NMB reversal.
These guys administered rocuronium, then sugammadex, then the patient developed severe anaphylaxis unresponsive to usual therapies, was in extremis, so they treated the patient with another dose of rocuronium.

Excellent talk by @AdamRutherford today @UniMelb . Eugenics was amplified through lies and fabrications.
Yet its founders, like Pearson and Fisher, also laid the foundations of modern statistics, which we still rely on for medical research. It’s a difficult truth to reconcile.
It will be important to understand why this is the case and how to remedy the difference.
Lignocaine is used across a broader spectrum of health practitioners and settings, and education definitely is a key component.
Deaths from local anesthetic systemic toxicity have declined for long-acting agents, but remain steady for #lidocaine. Findings highlight the need for broader education and vigilance in lidocaine use across specialties.

It'd be nice to know how this translates to the complications from short-term use of peripheral arterial lines in operative anaesthesia setting, inserted and removed shortly after the end of surgery.
8–12 million arterial catheters are inserted each year.
Non-infectious complications represent the greater burden of arterial catheter failure in ICUs.
This translates to up to 2.6 million non-infectious failures annually.

As you are taking your last breath, you would want to be in control of your final thoughts. This conclusion certainly raises important questions.
Patients on propofol in ICU often report negative & disturbing experiences, with distorted perceptions of reality, delusions & hallucinations.
Propofol is unlikely to achieve reliable unconsciousness and may contribute to a negative dying experience.
