🧬 Reduced oxaliplatin exposure | GI cancers | Meta-analysis
📚 24 studies (N=30,395)
➖ No significant survival difference
✅ G≥3 neuropathy OR 0.34
✅ G≥3 AEs OR 0.67
💡 Less oxaliplatin may improve the therapeutic index without survival loss.

The eternal question in solid tumor oncology, almost Shakespearean in its persistence
When is Cisplatin better than Carbo? When are they equivalent?
Excited to share our new paper in @JAMANetworkOpen where we examined the certainty of evidence of the 11 major meta-analyses (12 683 pts) on which guidelines based their recommendations
✴️Several efficacy variables have low or very low level of certainty
✳️Only high-certainty evidence signal was found for Cis>Carbo in ORR urothelial, and the no PFS difference Cis=Carbo in ovarian.
🟡Despite high heterogeneity impacting GRADE across tumour types, toxicities show known correlations
The story does not end here, highlighting the utility of meta-data appraisals and current evidence gaps. We might not need new trials, just greater access to data from completed studies

for the next 18-24 mo., the biggest bottleneck in AI drug discovery is experimental validation and verification
however it is increasingly clear that in 36 mo., the biggest bottleneck in AI drug discovery will be clinical trial scarcity
the challenge with US clinical trial infrastructure goes far beyond simply regulatory reform
in fact, outside of Australia, US FDA is the fastest in the world in getting INDs reviewed (30 day response time vs 60 in UK and Western Europe, and 60 working days in China)
Yet, median time to site activation after trial regulatory approval for oncology trials is:
US (academic oncology site) - 171 days (~6 mo)
Australia Phase 1 - 63 days (~1.5 mo)
China - as quick as 4 weeks
It gets worse. Based on July 2026 analysis, US sites not only take median 124 days to get sites activated, it takes *another* 93 days to get FPI (first patient in).
Australia sites can usually get from site activation to FPI in 2 weeks.
To top it off, it takes *309 days* on average for US trials to get 75% of sites in a trial activated
what's going on?
the challenge is multifactorial - in the US, drawn out budget negotiations at every clinical site, significant overlapping local ethics and protocol reviews (esp at high profile academic research centers), and extensive site contract negotiations lead to months / quarters / years of delays.
There is no magic bullet solution - no fell swoop of regulatory or policy change will address this - wholesale revamp is needed.
simultaneously, US and western biotechs (and frontier labs) can't just wait around for systemic change to unfold. They need to play the game on the ground in order to establish a human data edge
a new clinical execution paradigm + AI-native translational layer is sorely needed

🔥 Does adding immunotherapy to radiotherapy actually improve outcomes?
A new 2026 systematic review/meta-analysis evaluated radiotherapy + immune checkpoint inhibitors (ICIs) across cancers. (Taylor & Francis Online)
📌 Key message:
RT + ICI appears to improve antitumor efficacy and survival compared with RT-based approaches alone, supporting the biological synergy between radiation-induced immune priming and checkpoint blockade.
💡 Why it matters:
Radiotherapy may be more than a local treatment. In selected settings, it could potentially enhance systemic immune response and improve the effectiveness of ICIs.
⚠️ Limitations:
• Heterogeneous tumour types and RT schedules
• Variable ICI timing and agents
• Results should not be extrapolated uniformly across cancers
🩺 Clinical verdict:
PROMISING, but disease-specific evidence matters.
RT + immunotherapy should not be viewed as a universal strategy yet.
