It stands to reason that an intervention specifically designed to stimulate and train the immune system would engage deeply conserved evolutionary defense mechanisms.

"Together, these findings indicate that both prenatal immune perturbations induced by MIA and chronic postnatal peripheral immune activation can create a long-standing pro-inflammatory susceptibility state that primes abnormal CNS responses, driving downstream molecular and cellular pathology."

This week, I joined @NIHDirector_Jay, @DrOzCMS, and @DrSchwartzCDC at NIH to open a historic clinic dedicated to studying and caring for patients experiencing health problems following vaccination.
We must listen to patients and families who report vaccine injuries—not dismiss or gaslight them. President Trump and I are building a health system that takes them seriously, follows the evidence, and uses what we learn to make vaccines safer.
This is how we earn back trust in our public health institutions.
A brilliant deconstruction of the criteria we use to define profound autism, from Dr. Sylvia Fogel: @FogelSylvia
A new CDC study exposes how inadequate our autism data infrastructure is. The IACC Strategic Plan calls for better. It was adopted over a month ago. @HHSGov, it’s time to act. Advocates, raise your voices: these data shape the services and supports people need.

A subset of people diagnosed with severe psychiatric illnesses may actually have immune-mediated brain diseases. Properly diagnosing and effectively treating these patients is an important area of discovery in the mental health space.
I had a deeply interesting and informative time visiting researchers at the National Institute of Mental Health yesterday, who are working on cutting-edge approaches to that issue and many more.
“Findings indicate that neuroinflammation is a central mechanism linking ASD-related comorbidities with established [Alzheimer’s/dementia] risk factors.
The evidence suggests that reducing neuroinflammation, particularly through interventions targeting microglial activation during childhood or early adulthood, may lower the likelihood of early-onset [Alzheimer’s/dementia] in individuals with ASD.”
"Findings indicate that neuroinflammation is a central mechanism linking ASD-related comorbidities with established AD/ADRD risk factors" pubmed.ncbi.nlm.nih.gov/4281… I think we can all agree that the immune system has a bearing on some autisms...

Autism advocates,
It is important that we make a clear distinction when discussing “co-occurring conditions.”
There is a meaningful difference between medical and biologically based conditions that co-occur with autism - such as epilepsy, gastrointestinal disorders, immune dysfunction, sleep disorders, and metabolic abnormalities - and psychiatric or behaviorally defined diagnoses, such as anxiety, ADHD and bipolar disorder.
Many of us hear the phrase “co-occurring conditions” and assume an organization is referring to the former: the medical and biological conditions that significantly affect health & quality of life, and possibly drive the behaviors we call autism.
But in some cases, organizations are using the term primarily to refer to more lateral, phenotypic conditions.
That distinction matters.
We should be specific about what we mean by “co-occurring conditions,” because these categories raise very different research questions, require different forms of clinical evaluation and can lead to very different priorities for research and care.
Autism is extraordinarily heterogeneous, yet we continue to study causes and treatments as though it were a single, uniform condition.
Without meaningful subtyping, important biological signals can be diluted or obscured, and potentially effective treatments for specific groups may appear ineffective when evaluated across the entire autism population.
We urgently need biologically anchored subtypes that can help identify distinct mechanisms, trajectories, and treatment responses.
This is essential if we are serious about moving toward precision medicine and providing each person with care that reflects their individual biology and needs.
Perhaps the most important distinction of the new Strategic Plan is its focus on translation and accountability.
It is not enough to simply advance the science, the goal must be to ensure that meaningful discoveries reach the individuals and families who need them.
This plan creates a pathway for research to move from discovery to real-world impact, with accountability mechanisms to ensure progress does not stall along the way.
Ultimately, it brings greater urgency and accountability to turning scientific progress into meaningful improvements in the daily lives of people with autism.

JUST ANNOUNCED: HHS Secretary RFK Jr. and Vice President JD Vance will speak at the MAHA Summit tomorrow, Tuesday, September 29.
One of President Trump’s main focuses for this administration was to Make America Healthy Again.
Since January 2025, thanks to the work of @SecKennedy and the team over at @HHSGov, we have been fulfilling that promise to the American people, and the great MAHA movement.
Tune in tomorrow to hear more about the great work being done, and what is coming in the future to secure a stronger nation full of healthier Americans.

"Children born without ASD to FRAA-positive mothers prenatally supplemented with folinic acid remained ASD-free at preschool age" pubmed.ncbi.nlm.nih.gov/4279… Potentially really important. And "the single folinic acid-group ASD case was attributable to a pathogenic SHANK2 variant"
Once you realize that even talking openly about regression, despite regression being documented in a substantial subset of children with autism (30-40%), can be met with dismissal and skepticism, it adds an entirely new layer to this conversation.
When so many parents believe they witnessed a specific trigger preceding a profound change in their child, their observations deserve to be heard and studied rigorously, not mocked, dismissed, or treated as though asking the question itself is unacceptable.
What are we even doing?
These are children. Their experiences matter. Their parents’ observations matter. And getting the science right matters.

Who reviews grants shapes what gets funded. Research expertise alone has not delivered the kind of translation that reduces suffering and morbidity in autism. Clinical care and lived experience are essential. The adopted IACC 2026–2028 Strategic Plan calls for bringing both into the grant review process.
NIH’s peer review process depends on the participation of experts across the research community.
We’re seeking scientists with expertise in human-based research approaches to serve as NIH grant reviewers.
If that’s you, I encourage you to bring your perspective to this important work.

The immune system is inextricably and reciprocally linked to the functioning of the brain and nervous system👇🏼
The immune system is inextricably and reciprocally linked to the functioning of the brain and nervous system. This honestly shouldn’t even be news anymore. But this framework has not penetrated our systems of clinical care and should be foundational in research paradigms investigating brain symptoms and chronic illness.
