🫀Is there still a role for low-dose #digoxin in modern HF therapy?
Primary results from the #DECISION trial presented by Prof. Dirk van Veldhuisen at #HeartFailure26:
📍Low-dose digoxin did not significantly ⬇️ the primary composite endpoint of CV mortality + total worsening HF events
📍However, there was a non-significant 24% relative ⬇️ in worsening HF events
📍In the as-treated analysis, digoxin was associated with a 34% relative risk ⬇️ in the primary outcome
📍Low-dose digoxin was generally safe & well tolerated, including in women & patients with #AFib
📍High study-drug discontinuation rates ➡️ reduced statistical power

A memorable evening at #HeartFailure26 celebrating excellence, leadership, & legacy in cardiovascular medicine🫀
The prestigious HFA lectures honoured two remarkable figures:
🫀 The HFA Philip Poole-Wilson Lecture delivered by Prof. Karen Sliwa
🫀 The HFA Eugene Braunwald Lecture delivered by Prof. Frank Ruschitzka
Congratulations as well to both awardees on receiving the Heart Failure Association Lifetime Achievement Award, recognising decades of scientific leadership, mentorship, & dedication to advancing heart failure care worldwide.

🫀In #advanced HF, how do outcomes compare between #HM3 #LVAD & #HTx strategies in middle-aged & older patients?
Presented by @NirUrielMD at #HeartFailure26:
📍When survival was measured from treatment, HTx showed higher 2-year survival vs HM3 LVAD in both age groups
📍However, when waitlist time was incorporated, HM3 LVAD therapy outperformed transplant listing in patients aged 50-64 & was numerically favorable in older adults
📍These findings highlight the importance of evaluating real-world treatment pathways, not transplant outcomes alone
📍The role of durable LVAD therapy continues to expand as bridge-to-transplant & destination therapy in carefully selected patients

Despite strong evidence that #GDMT improves survival in #heartfailure, real-world adherence remains suboptimal
🫀⚠️Improving adherence requires a multi-layered strategy:
📍Clinicians: routine adherence screening, MDT, simpler regimens
📍Health systems: EHR-integrated monitoring, standardise therapeutic protocols & ⬇️ costs
📍Patients & caregivers: stronger support, education, empowerment & digital tools

New statement from #ESC_HFA: #GDMT saves lives in #heartfailure but medication non-adherence remains a major barrier. We call for fixed-dose combinations, digital monitoring, enhanced patient education & multidisciplinary care to bridge the gaps! @escardio @HFA_President
