When a Medicare patient doesn't pay their deductible or coinsurance at a hospital, Medicare pays the hospital back 65% of it.
When the same patient doesn't pay my office, Medicare pays me nothing.
Why is the hospital's bad debt Medicare's problem and mine isn't?

This is what I can’t stand about modern academic medicine.
Too many won’t stand up for fellow physicians but are perfectly happy with consolidation as long as it’s under their institution’s banner.
They consider themselves “above the fray” when their colleagues are fighting to preserve their practices and independence. But they have no problem inserting themselves into identity politics.
Dirtbags… the lot of them

You have Free Will. Why do you want to deny the Doctors Free Will just because you want to use your Health Insurance? We are parents and have children. We are people like you. Sounds INHUMANE.
Sounds like you have a Health Insurance problem not a Doctor Distribution problem. If Your Health Insurance treats Doctors so poorly they are quitting perhaps your problem is with your Health Insurance.
Perhaps you’re looking at it all wrong. The Doctors are finding a SOLUTION to YOUR Health Insurance problem.
You did nothing to help us. We are helping ourselves and our children. We have a right to spend time with our children and not work like a slave for YOUR Health Insurance.
You have a right to use your Health Insurance. We have the Right to walk away from a MISERABLE Job.
Direct Specialty Care and Direct Primary Care is saving careers and lives. Doctors are choosing the Free Market rather than quitting medicine entirely.

A physician converting from insurance participation to cash only hasn’t merely created an additional payment option. For the patients who previously used their insurance there, one option disappeared. That’s perfectly within the physician’s rights, but it has distributional consequences. If this occurs at scale, as Lighthouse actively wants it to, then patients who prefer comprehensive insurance can have increasingly comprehensive coverage while having fewer participating clinicians from whom to purchase care. That’s patient abandonment. 🤷
You know what should be automated by AI in the US? PORTS. Not fucking doctors. Unionize

How about we all just fucking go on strike it’s getting to be insane. You can’t really tell me that a brand new fucking knee installation is cheaper for society to pay for than a tankless hot water heater or a whole house generator. You can’t tell me the 2 hours you put in is worth less than a boat mechanic’s hourly rate!!! It is so fucked at this point it’s time to disengage with the payers.

This is exactly what’s wrong with the entire system. Why is Medicare, a government program, used as the benchmark for physician reimbursement?
Medicare physician payments have declined roughly 33% after inflation since 2001, while the cost of providing care continues to rise.
In what world would physicians, given a choice, allow their reimbursement to be dictated by Congress—a body compromised by special interests, beholden to lobbyists, and perpetually fighting for reelection?
We’ve allowed politicians, whose decisions are shaped by political contributions and lobbying, to set the benchmark for our pay.
And where is the evidence that reducing physician reimbursement actually lowers insurance premiums?
The entire arrangement is absurd
Nibbling at the edges is futile. Until physicians realize this fact, we’ll keep arguing our value based on what government can do for us.
You continue to be disingenuous….
First, Lower payments to a small
Subset of physicians do not guarantee lower premiums for individual patients or even over all cost. History, shows that’s a fundamentally dishonest statement
Second, the bill federally forces rates and pay on physicians that haven’t accepted or agreed to them. The rate would effectively be determined by the payor and remove any capacity to question it.
Third, the Insurers have already used QPA rules that included ghost rates and excluded bonus and incentive payments. The Fifth Circuit found that those rules artificially lowered the benchmark and struck down those provisions.
even with corrections to the formula, there would be little to no recourse for manipulation of those numbers.
Furthermore, eliminating arbitration would weaken independent practices’ negotiating position by removing an independent federal process for challenging inadequate reimbursement.
Physicians would face an in-network payment benchmark without an in-network agreement. Insurers would gain more leverage over reimbursement, and physicians would lose a means of challenging payments that fail to account for the care provided.
Essentially weaponizing federal regulations against independent physicians

That a doctor should have any right, desire or choice in the matter, was regarded as irrelevant selfishness; his is not to choose, they said, but ‘to serve.’
That a man’s willing to work under compulsion is too dangerous a brute to entrust with a job in the stockyards — never occurred to those who proposed to help the sick by making life impossible for the healthy.
I have often wondered at the smugness at which people assert their right to enslave me, to control my work, to force my will, to violate my conscience, to stifle my mind — yet what is it they expect to depend on, when they lie on an operating table under my hands?
Their moral code has taught them to believe that it is safe to rely on the virtue of their victims. Well, that is the virtue I have withdrawn. Let them discover the kind of doctors that their system will now produce.
Let them discover, in their operating rooms and hospital wards, that it is not safe to place their lives in the hands of a man whose life they have throttled. It is not safe, if he is the sort of man who resents it — and still less safe, if he is the sort who doesn’t.”

It is not safe, if he is the sort of man who resents it — and still less safe, if he is the sort who doesn’t.”

I will not be called a healthcare hero out of one side of these goblins’ mouths and then get 🍇ed by a thousand cuts while hospital admins, medical device grifters and supplement dirtbags like Oz gut the last bit of actual enterprise in medicine by killing private practice and then coordinating a fake& gay victory lap, making all the money and leaving nurses and doctors broke, healthcare materially worse and everyone baffled as to why they’re fucking over their boomer constituency (not that I care. They deserve it at this point). If you are actively choosing to systematically strip any chance of wealth building from some of the last of the capable caregivers you’re going to be abandoned when the SHTF. We have already taken a 70% cut against our less effective boomer forebears in the 1980s who worked way less comparatively and had an objectively better life.
Back in Hammurabi’s age we didn’t have to fucking pay taxes and doctors were ACTUALLY butchers in that age. What can be done now is nothing short of miraculous.
I would rather twirl in a fucking office chair and sell insurance all day with my cock in my hand or sell medical devices on zoom or hock vitamins on X like a little Dr. Oz LARP with no fucking pants on eating DoorDash all day. Reap what you sow, dipshits. Let your lungs fill with fluid when the big one comes. I have no compassion for your generation’s stance towards me or my desire to exchange ALL of my free time and quality of life, where you are getting old and shitfu yet wish to take away any chance to give my kids an advantage by making me have an income less than the quack chiropractor up the street.
When you feed the seed corn to the pigs of the fundamentals of society (cops, doctors, fire, yes… even lawyers) you will start seeing a breakdown in societal trust and competency. High sacrifice careers MUST be compensated commensurate to the risk. If you don’t you will get what you pay for for those services on a societal level (cops who shoot you and are retarded, doctors who are unqualified/dangerous/worried about their poor finances/poor free time/unfocused on you and also retarded, lawyers who are overworked/worried about their finances/not focused on you and retarded.
As a degenerate who has burned through a quarter trillion tokens I can safely say Chat GPT will not bail you out of this shit either.
GOOD LUCK BOOMER SCUM

Give Dr. Oz credit on this one. This is what I voted for. Saving people money and cutting unnecessary waste.
Here is what has been going on. Medicare has been paying twice for overlapping work when someone has an office visit and a procedure on the same day. Those extra costs can eventually mean higher premiums and more out of pocket costs for patients.
@DrOzCMS is pushing a fix so Medicare is not paying full price twice for the same overlapping work. This is the kind of change I want to see in healthcare

This is why Republicans keep losing the healthcare argument.
Shit takes from people who don’t understand the first thing about how healthcare is actually delivered.
A patient walks into a physician’s office.
The physician evaluates the problem.
The physician determines a procedure is necessary.
The physician can safely treat the patient right there, that day, in the lowest-cost setting.
And Buzz Patterson calls that a “healthcare rip-off.” Seriously? What would Dr. Oz and Buzz the healthcare expert prefer?
Yes, let's send Grandma home?
Tell her to come back next week?
Schedule another appointment?
Another drive.
Another train ride.
Another copay.
Another day off work for her daughter. Another opportunity for her condition to get worse.
Solid strategy.
Medicare already has longstanding multiple-procedure rules where, in certain surgical circumstances, the highest-valued procedure is paid at 100% and additional procedures are paid at reduced rates.
That is not what we are talking about here.
CMS is now proposing to apply that logic when a physician provides a separately identifiable E/M visit and a procedure on the same day.
The most expensive service gets paid at 100%.
The other gets cut to 50%. Centers for Medicare & Medicaid Services
Think about what that means.
The patient comes in with a problem.
The physician evaluates her.
Finds the problem.
Can safely fix it immediately.
And CMS says: great efficiency, we’ll pay you less.
That is backwards.
If the physician sends her home and brings her back later, we have preserved two separate encounters.
If the physician solves the problem today, CMS says there is “overlap” and proposes reducing payment.
And somehow The great wizard of Oz and his sidekick Buzz have turned that into physicians “double billing.”
It isn’t double billing! These are separately identifiable services.
CMS itself describes the policy as an attempt to account for what it believes are efficiencies and potentially duplicative resource valuation when an E/M and global procedure occur together.
Fine. Debate the valuation. Stop pretending a physician evaluated a patient twice!
And while we are suddenly so concerned about waste, perhaps Dr. Oz and Buzz could look at the gigantic payment distortion sitting directly in front of him:
site of service.
MedPAC has been documenting this for years.
Its own 2024 example:
Removal of skin lesions in a clinician office: $165
Same service category in a hospital outpatient department: $404
Same patient.
Same basic service.
Different sign on the building.
$238 more.
So let me understand the healthcare reform strategy:
Physician evaluates you and treats you safely in the office on the same day: Cut one of the services 50%.
Move care into a hospital outpatient department:
Medicare may pay dramatically more.
And these guys think they found the rip-off.
Incredible. This is exactly how we create incentives for care to migrate into higher-cost settings.
The independent physician who can diagnose and treat the patient in one visit gets squeezed.
The hospital outpatient department carries a much richer payment structure.
Then Washington wonders why independent practices disappear, insurance premiums rise, and everything ends up owned by a tax-payer subsidized hospital system.
@DrOzCMS, this is the healthcare problem sitting directly in front of you.
Fix site of service.
Stop financially rewarding care simply because it happens inside an HOPD.
Reward physicians who can safely diagnose and treat patients in their offices.
Reward lower-acuity care. Reward efficiency.
Reward the doctor who can fix the patient in one damn visit. Don’t punish it.
FYI, if you'd have called 10 independent practices, they would have nicely told you this in 60 seconds. But you retards always think you know better.
Stop harming patients!

with ai it’s endless drift. long horizon is all experimental at this point, it’s like the ai is the guy from memento. the game is putting those collars on the ai forcing it to prove and check, and get infinitely more granular with the controls over and over. it’s never really a goal to achieve, it’s more a system that we just inch forward a percent at a time.
if you’re deep in ai feeling frustrated that it won’t do exactly the thing despite the harness engineering and data cleaning and tool building, well i think this phase of the technology is not so much about actually getting that nailed down.
just going as far as we have we’ve reached this sort of event horizon with ai. it’s near impossible to fully catch up from the outside without unhealthy obsession with it.
