EASi-KIDNEY trial (vicadrostat in CKD) stopped early. "Trial data did not support a meaningful expectation of clinical benefit". Cardiovascular trials of EASi program continue. boehringer-ingelheim.com/hum…

Very excited to share our new work in @jclinicalinvest!
How do anti-PLA2R autoantibodies (abs) actually cause podocyte injury in membranous nephropathy (MN)?
Great international collaboration with @KuChengLung @UKEHamburg & @HCKH_Hamburg.

Clinical phenotypes and etiologic subtypes of thrombotic microangiopathy in SLE
🧐SLE-TMA is frequently limited to the kidney requiring biopsy for diagnosis. Kidney phenotype identifies subgroups with distinct trajectories
▶️academic.oup.com/ndt/article…
5/11 But SLE-TMA is not a single diagnosis—it is a clinical syndrome with different underlying mechanisms.
In our cohort:
🔹 ~5% had a TTP-like phenotype
🔹 ~35% had antiphospholipid syndrome nephropathy
🔹 ~60% had suspected complement-mediated TMA
10/11 Important limitations:
This was a retrospective study, without systematic ADAMTS13 activity or complement testing.
Access to complement-inhibiting therapies was also unavailable, and no patient in this cohort received these treatments.
11/11 Main message?
TMA in #Lupus is heterogeneous, frequently kidney-limited, and associated with poor long-term kidney outcomes.
Recognizing the underlying phenotype/mechanism matters.
Congratulations to all authors! Much more work is needed to improve outcomes for our pts.
1/11 🧵 I invite you to read our publication on thrombotic microangiopathy in #Lupus.
This large cohort (n=124) highlights the high frequency of a renal-limited phenotype & differences in presentation and outcomes according to TMA etiology. shorturl.at/CGwuV @NDTsocial
3/11 SLE-TMA usually occurred after a long history of disease: median time from SLE diagnosis was 62 months, and 94% of patients had a history of previous kidney involvement.
Thus, for most patients, TMA developed in the context of established SLE and kidney disease.
2/11 “How did you get 124 patients with SLE-TMA? Something must be wrong.”
Unfortunately, no. #Lupus represents a major burden in Latin America. As shown in the figure, SLE-TMA occurred in ~7% of our #LupusNephritis population.
In fact, this week alone we saw 3 additional cases.
These PRO vs. CON discussions are a great way to critically evaluate emerging tx strategies. I recommend this debate on combination ("triple") therapy for #lupusnephritis. Ultimately, the goal is to define the best personalized treatment for each patient shorturl.at/lxNOl

Along with the FDA investigation into avacopan over alleged data manipulation after unblinding and reports of hepatotoxicity, the @NEJM has retracted the original clinical trial publication. Hopefully we will find other tools to reduce steroid exposure in vasculitis
