Presented today at #ACC24:
In the DanGer Shock trial involving patients with STEMI and cardiogenic shock, mortality at 6 months was lower with mechanical circulatory support with a microaxial flow pump than with standard care alone. Full trial results: nej.md/3xyXU3t

We’ve seen it in clinical practice for years. Now there’s randomized data showing benefit to LV unloading in #cardiogenicshock.
Congratulations to Dr. Jacob Møller & colleagues for today's #ACC24 Late-Breaker & simultaneous @NEJM publication of DanGer Shock! What a win for the interventional community & our patients with AMI Cardiogenic Shock! nejm.org/doi/full/10.1056/NE…

These are fantastic images, @Babar_Basir
Very clearly shows the mechanism of traveling of the knot.I generally use this to my advantage with closing MCS cases by threading a micro sheath over the last remaining 0.035” wire,to take a pic of the CFA before cinching it completely. – at Gainesville, GA
Replying to @Babar_Basir
Tautening, traveling of perclose knot is critical to achieving hemostasis with minimal to no bleeding. When using MCS in shock, these same principles are needed to limit risk of complications. Hence in my opinion almost every case should be pre-perclosed if possible.

What are the mechanisms of LV unloading on #VAECMO in #AMI? Our research published @JACCJournals highlights 1) importance of timing of unloading for PVA reduction, 2) improved cardioprotective signaling and 3) infarct reduction in a preclinical AMI model with #ECPella vs ECMO.
