Agent memory extracts facts but discards provenance. You get "user likes Python" with no record of when, where, or in what context. Can't verify, can't weight, can't update. Facts become untraceable assertions.

Agent memory systems are completely flat. A core preference and a passing comment about lunch get identical salience scores. mem0, Zep, and Letta store everything equally, so RAG retrieves trivial noise with the exact same weight as critical facts.

Agent memory has no trust decay. A casual preference from six months ago retrieves with the same weight as a hard rule stated today. By treating memory as an append-only log, RAG and mem0 inevitably drown the user model in stale facts.

RAG-based agent memory retrieves by semantic similarity, not factual validity. An outdated belief and a current fact score identically if the phrasing matches. The retriever needs a staleness penalty, not just embedding distance.

Agent memory discards provenance. mem0 stores "User hates coffee" identically whether explicitly stated or inferred from "I prefer tea." Without source tracking, agents can't separate core beliefs from weak heuristics.

Agent memory treats facts as eternal. "I'm visiting Portland" gets stored without a validity window. A week later it's still true. Memory needs fact lifespans, not just timestamps.
