#IDWeek2026 Lastly, What's Hot: HIV Basic Science, bench and translational data.
Broadly neutralizing antibodies (bNAbs) can be utilized from elite controllers as a potential ART or LT control? Half life of 100d makes them attractive. RCT showing prolonged ART-free control.
Post-intervention controllers may be big targets for immune therapies, treatments, and possibly HIV vaccination prevention with animal studies.
#IDWeek2026 Third, What's Hot: Clinical HIV, with focus on presentations at a conference. Tough with HIV cuts nationally.
Starting with ISL/DOR treatment naïve and switch data, islatravir at a lower dose with no lymphopenia AE and showing equiv to Biktarvy.
Purpose 1 OLE and PURPOSE 2 OLE showed twice yearly LEN for PrEP is a transformative option for cisgender women and more.
LA CAB/RPV at week 96 showed SUPERIORITY to oral ART and highly preferred in adolescents. HIV ART for the future will head this way.
Once weekly ISL/LEN vs once daily Biktarvy showed good equivalence. Questions maybe on missing doses on qweekly ART and resistance. Also no Hep B coverage with ISL/LEN.
Very new trial (6 days ago) in NEJM with the IMPALA study across Africa with no resistance testing done for HIV ART in settings of high experience of NRTIs. LA CAB/RPV q8weeks showed non inferiority to daily oral DTG backbone ART.
More HIV data on switch to ISL/DOR and Bictegravir combination data with lenacapavir in a single tablet regimen.
#IDWeek2026 Next up What's Hot: Basic ID Science, papers over past year that are 'hot' and active.
Starting with Cefazolin inoculum effect paper, showing no diff in mortality with CZIE vs none, but microbiologic eradication was higher without.
(fingers hurt, going slower)
C. Diff colonization in mouse models and early human infant data shows that early colonization (unclear infection) remodels epithelial gut layers that persist past colonization compared with none. Maternal mouse vax prevents. Colorectal CA tie-in?
Mucosal vaccination against C. diff colonization in a mouse model showed that rectal administration may inactivate toxin and promote clearance faster by day 9. Rectal vaccine plus adjuvant plus inactivate toxins prevented colonization!
CDI antibiotic study showing high risk with Amox-Clav (clavulanate removal is less than 3 compared to above 10 with), Clindamycin, cephalexin, cipro, pantoprazole, etc. Somewhat surprising low spectrum causes, but barely a majority of CDI attributable to abx, other factors there.
#IDWeek2026 Starting What's Hot: Clinical ID Science, papers over past year that are 'hot' and change practice
Starting with REMAP-CAP
NTM very tough infection to treat, especially M. abscessus. But possibly Omadacycline is a new/old-new option that can help treat these infection as MONOTHERAPY.
