Great to finally see the results of the #PROTEUS substudy comparing #perioperative Apa+ADT with the true SOC (RP w/o neoadjuvant treatment) - which should at least reduce some concerns related to the active comparator with ADT in the main study that didn’t reflect what we actually do as a standard in HR patients #ESMO26
Outcomes in all #endpoints significant improved
✅EFS: HR 0.38, 95% CI 0.26–0.55
✅MFS: HR 0.49, 95% CI 0.26–0.90
✅TT 1st Sub Rx: HR 0.29, 95% CI 0.20–0.43
The question remains which of these outcomes are most clinically relevant and meaningful #PROTEUS #ProstateCancer #UrologicOncology #Apalutamide #RadicalProstatectomy @oncodaily @OncoDailyGU @urotoday @Albert0Briganti @JNJInnovMed

🚨 New in @JUrology: "Language as a powerful influencer in #ProstateCancer" 🚨
✒️ New essay w/ @uroegg & Jessica Rett @LinguisticsUcla
📄 Words are neither neutral nor fixed
♋️We use prostate cancer, a disease spanning indolent to lethal under one name, as a case study:
👉 MRI "lesion"
👉 clinically "significant" disease
👉 "positive" margin
❓ We all know these terms shape how patients hear us
🧵 5 strategies you can use today 👇

An 8 year labor of love to get this study complete but it's finally here-- a comprehensive longitudinal assessment of >250,000 @VeteransHealth patients with de novo #microhematuria, details on their cystoscopy evals, and risk factors for MH-related bladder cancer diagnoses.

Cochrane on HoLEP: 52 RCTs, 6,242 men, no external funding, search to April 2026.
Vs TURP at 12 months: similar symptoms, QoL, major AEs, retreatment, erectile function. One clear win: transfusions, RR 0.19.
I do HoLEP. I accept this. Thread.

🚨 Half of focal therapy for #ProstateCancer in the US is happening where no guideline supports it 🚨
@JAMA_current research letter
👥 1,179,384 pts w/ nonmetastatic PCa, NCDB 2010 to 2023
📊 15,672 (1.3%) got focal therapy
⚠️ 51% was in low, high, or very high risk disease.
-No guideline supports routine use in ANY risk group outside trials or registries
⚠️ Low risk held flat (1.8% to 2.2%) and favorable intermediate is rising (2.1% to 2.9%) while marketing and&reimbursement expand
⚠️ Focal therapy more likely w/ age ≥75 (adjusted probability 22.9%), community centers (7.6% vs 2.5% academic), nonprivate insurance
✒️From the accompanying editorial
"Volume of this kind shows diffusion, not benefit."
🎯 Key points
1⃣In low risk this converts surveillance candidates into procedure pts
2⃣In high risk it is undertreatment.
3⃣Repeated use is building "an aura of legitimacy the data have not earned." We owe pts trials, not marketing.
