Daraxonrasib = $500K/QALY. Agree CE analyses challenging w poor prognosis dz: even doubling OS adds only 0.65 life-yrs.
Moving earlier may yield more life-yrs, but at ~$40K/mo, longer tx keeps $/QALY high.
Interesting scenario w impressive drug, but $ above most CE benchmarks.
Alright. As promised. Here is our preprint assessing the cost-effectiveness of daraxonrasib!
Daraxonrasib nearly doubled median overall survival in previously treated metastatic pancreatic adenocarcinoma, in which median survival with standard chemotherapy was about 7 months, and should remain available to eligible patients.

Thought experiment:
Let's take a very simple hypothetical trial involving 100 patients in the APA arm and 100 patients in the ADT alone arm. Pulling from PROTEUS EFS data, assume that, by 5 years, 60 patients in the ADT arm have had a BCR compared to 50 in the ADT+APA arm.
Assume they all get imaging at around the same, low PSA. A 2025 ASCO GU abstract of 217 post-RP patients found that at comparable median PSA values (~0.23–0.25 ng/mL), 52.7% of PSMA PET scans were positive versus only 9.9% of conventional imaging studies.
Using those probabilities of detection, assuming M1 in all detected cases, and the differential PSMA PET utilization from the paper (~64% in the APA arm and 72% in the ADT arm), the MFS benefit at 5 years would be approximately 5.3%.
If PSMA PET was utilized equally in each arms (72% in each), the MFS benefit at 5 years would be approximately 3.5%.
Math in the excel screenshot below :)
Would this reduction in MFS benefit (delta of deltas= 1.4%) tip these hypothetical results from significant to non-significant? To me, this would be an interesting sensitivity analysis to run. #ASCO26 #radonc #medtwitter #pcsm

❗️ Hot take Friday: Precision oncology is the biggest narrative scam in cancer medicine. “Find the right gene and we’ll cure cancer.” We’ve been hearing this for 30 years. Meanwhile, surgery and radiation are still the only things that actually cure solid tumors. Let’s go 🧵

We operate.
We ablate.
We radiate.
Because doing more feels like caring more.
But this trial is a reminder: intent is not evidence.
The bravest thing in oncology is sometimes saying: less is more!

Imagine 4 new anticancer drugs Pallituzumab, Geriatriximab, Symptomab, & Exercizumab hit the market. They should dominate the plenary sessions at ASCO & command billion-dollar revenue. But they don't because they're nonpharmacologic & shame on us.

We’ve seen this movie before over the past 2 decades. Trying to replace curative (RT) treatment with non-curative drugs. It’s never ended well, at least not yet.
No wonder the public have misconceptions about radiotherapy when you see a paper title such as this in @TheLancetOncol
Shouldn’t it be Chemo-immunotherapy alone or without surgery or radiotherapy?
