DLL3-targeted TCEs, from the first results in relapsed SCLC to the trials now testing them in first line: Solange Peters takes this on at our ESMO 2026 symposium, Sunday 25 October from 13:00. Send her your question ahead at grillspeakers.com. We hope to welcome you in the Salamanca Auditorium, Hall 10, so add it to your calendar if you have not yet 🙂

Checkpoint inhibitors are themselves changing. Bispecific antibodies against the PD-1 or PD-L1 axis and VEGF appear to be showing benefit over PD-1 or PD-L1 inhibitors alone, and @Fskoulidis, of @UTMDAnderson in Houston, is waiting on the final trial results before calling a change in first-line chemoimmunotherapy.
Behind those he puts ADCs moving into the first line alongside checkpoint inhibitors, and TCEs, where the progress made in SCLC has still to be shown in NSCLC. How the newer immunotherapies then combine with targeted therapy is the question he leaves open.
Recorded at the recent Swiss Oncology Summer School in Gruyères.

After first-line treatment in SCLC, the choice now depends on time to relapse, prior therapy and tolerance, with ICIs, DLL3-targeted TCEs and ADCs in play. Noemí Reguart takes on sequencing at our ESMO 2026 symposium, Sunday 25 October. Send your case ahead at grillspeakers.com. We look forward to seeing you in the Salamanca Auditorium, Hall 10, at 13:00 🙂

T-cell engagers and ADCs in SCLC: which patients gain from each, in which line, and with what toxicity when one follows the other? Nikolaj Frost of Charité – Universitätsmedizin Berlin takes up those questions at ILCS 2026 in Lausanne on 13 November.
We would be glad to welcome you. Join him in Lausanne or online, and find his 15:30 slot on the programme.

A DLL3-targeted TCE has improved overall survival over chemotherapy after first-line treatment in SCLC, consolidation ICI has entered practice in limited-stage disease, and ADCs are in trials. Martin Reck opens the talks at our ESMO 2026 symposium on Sunday 25 October. We would be delighted to see you. We start at 13:00 in the Salamanca Auditorium, Hall 10.

ICIs with chemotherapy are the standard in extensive-stage SCLC, yet only some patients keep their benefit. Every newer option is measured against that.
Martin Reck opens the talks at our ESMO 2026 symposium on Sunday 25 October. The programme is on the page below. We would be delighted to see you, so come to the Salamanca Auditorium, Hall 10, a few minutes before 13:00.

Two first-line combinations in EGFR-mutant NSCLC have lengthened progression-free survival over a third-generation TKI alone, with more side effects. In ALK-positive disease, a third-generation inhibitor has shown long-lasting control, including in the brain.
Hidehito Horinouchi (@HHorinouchi), Japan, speaks on the new standards at the International Lung Cancer Summit (ILCS) 2026, in Lausanne on 13 November, at 10:10 CET. Come and meet him at CHUV, or join online. We look forward to welcoming you in Lausanne.

A commercial molecular report runs long, and the risk @ChristianRolfo, of the @OSUCCC_James in Columbus, names is a clinician who reads the one targetable alteration and stops there.
Co-mutations carry information about the course of the disease. Some go with a shorter PFS; others mark a tumour more likely to transform as its mechanism of resistance. Reading them has to be taught, and the teaching he describes reaches beyond doctors into molecular nursing.
The report also has to connect an annotation to a drug, and he warns that the match needs care: it corresponds to what is in the present literature, and some cases escape the literature. A molecular tumour board is what interprets the rest.

Plasma genotyping is accepted for finding driver alterations at diagnosis. ctDNA to detect residual disease after curative treatment, and to guide adjuvant therapy, is still being tested in prospective trials.
Millie Das (@DasMillie11), USA, sets out where ctDNA stands at the International Lung Cancer Summit (ILCS) 2026, in Lausanne on 13 November, at 09:50 CET. Come and meet her at CHUV, or follow her talk live online. We would be very happy to see you there 🙂

The Swiss Oncology Summer School is organised independently of any pharmaceutical company, and @MdCurioni names that as the condition for two things at her own school: a free hand in the programme, and a free hand in the choice of speakers.
The faculty comes from around the world with one scope, in her words, which is to teach. For the days it runs, the meeting is the students and the faculty and no one else.

After earlier treatment of KRAS G12C-mutant NSCLC, a newer G12C inhibitor has been tested head to head against the first ones, and Ferdinandos Skoulidis presents the phase 3 data in a Presidential session at ESMO 2026 on 25 October.
Christine Lovly (@christine_lovly), of City of Hope, speaks on (K)RAS at the International Lung Cancer Summit (ILCS) 2026, in Lausanne and online on 13 November, at 11:20 CET. We hope you can be there for it 🙂

DLL3-targeted TCEs and new ADCs have entered SCLC treatment, and the order and combination of these agents are still open questions.
Solange Peters, of CHUV, chairs our ESMO 2026 symposium this Sunday and looks at this closely in her talk. We start at 13:00. Come and meet her in the Salamanca Auditorium, Hall 10. Got a case for her? Send it ahead at grillspeakers.com. We hope to see you in Madrid.

Most cytokine release syndrome with DLL3-targeted TCEs comes in the first cycle, a different pattern from the immune-related events of ICIs. Giannis Mountzios speaks on managing TCEs at our ESMO 2026 symposium, Sunday 25 October from 13:00. We would like to see you in the Salamanca Auditorium, Hall 10. Questions can go in ahead at grillspeakers.com.

Testing paid for by a drug company is a great opportunity, in the words of @ChristianRolfo, of the @OSUCCC_James in Columbus, and not enough on its own.
The route he sets out for the future is a cheaper one: digital pathology software identifying an alteration from a scanned slide in minutes, with a PCR, which does not cost what NGS costs, to confirm it. A slide scanned in rural Ohio or in a remote part of Brazil could then be read centrally.
The second barrier is the drug. In some countries a patient goes through a legal process to obtain a treatment for a specific alteration, and even where advocacy groups are organised by alteration, retesting at recurrence is not happening at the rate it should.
We filmed him at the recent Swiss Oncology Summer School in Gruyères.

The next frontier @FSkoulidis, of @UTMDAnderson in Houston, names in his own field is bringing targeted therapy earlier: into previously untreated, first-line metastatic KRAS-mutant lung cancer, and into combination with immune checkpoint inhibitors, both effectively and safely.
Registrational trials are enrolling now. The phase 2 data, combining KRAS G12C inhibitors with checkpoint inhibitors, produced responses he describes as deep and durable, and that is the basis he gives for expecting approvals of the combination in the foreseeable future.
Recorded with him at the recent Swiss Oncology Summer School in Gruyères.

With DLL3-targeted TCEs, cytokine release syndrome comes mostly in the first cycle, and neurological events need their own watch. Managing them differs from managing ICIs.
Giannis Mountzios, of Henry Dunant Hospital Center, Athens, goes through daily practice at our ESMO 2026 symposium, Sunday 25 October from 13:00. Medicine is studied differently from one country to the next, and access to treatment differs with it.
That is the reasoning @MdCurioni gives for drawing the Swiss Oncology Summer School's students from around the world. A network built across those differences lets a student think past the boundaries of their own system, and a group this small, as she reads it, might be what allows such a network to form at all.
She also puts the school forward as a model for other countries that will not have enough oncologists of their own.
