Could bedside ultrasound uncover a congenital anomaly missed before birth?
When a newborn presents with significant respiratory distress, the underlying cause isn't always immediately apparent. In this case, lung POCUS revealed an unexpected finding: liver tissue within the right chest, raising suspicion for a right-sided congenital diaphragmatic hernia. The diagnosis was later confirmed through additional imaging, highlighting how bedside ultrasound can help clinicians recognize unexpected pathology and guide early clinical evaluation in the NICU.
This article dives into…
🔹 How lung POCUS helped identify a congenital diaphragmatic hernia that went undetected prenatally.
🔹 The key sonographic findings that pointed toward liver herniation into the right chest.
🔹 The potential of bedside ultrasound as an early diagnostic tool in neonates presenting with respiratory distress.

An 84-year-old male presenting with progressive exertional shortness of breath and lower extremity edema was referred to Cardiology with concern for heart failure. He subsequently underwent point-of-care ultrasound (POCUS) assessment, which revealed several unexpected findings - including ascites, a hepatic mass, and abnormal tissue involving the inferior vena cava.
These bedside findings prompted urgent CT imaging, which revealed hepatocellular carcinoma (HCC) with extensive vascular invasion, tumour thrombus extending through the IVC into the right atrium, and pulmonary emboli.
➡️ Figure 4 shows the CT findings.
➡️ Figures 3 and 2 highlight the major abnormalities seen on POCUS.
➡️ Figure 1 shows the more subtle finding of ascites encountered during the initial examination.
Are you able to correlate the CT findings with the POCUS scans?
Together, the images illustrate an important strength of POCUS: while the examination should remain focused and clinically driven, being attentive to unexpected findings can dramatically redirect an assessment and accelerate further investigation.

A laceration near the Achilles tendon may look superficial…but is the tendon actually intact?
In this pediatric case series, point-of-care ultrasound (POCUS) was used to evaluate two posterior ankle lacerations where Achilles tendon injury was a concern.
In one case, bedside ultrasound demonstrated an intact tendon, helping exclude a deeper injury (see Figure 1). In the second featured case, POCUS revealed an Achilles tendon laceration with an associated hematoma (see Figure 2), helping clinicians recognize the injury and involve orthopaedic surgery.
This article also provides a practical approach to scanning the Achilles tendon, including probe positioning, imaging planes, and dynamic assessment (see Figure 3)!
⏩️ Swipe through to review the scanning technique, then compare an intact Achilles tendon with the sonographic appearance of tendon injury.
🩺 Clinical takeaway: POCUS provides a rapid, dynamic way to visualize the Achilles tendon directly at the bedside and may be particularly useful when an overlying wound makes tendon involvement uncertain.

Cosmetic procedures like non-surgical nose reshaping with PDO threads are becoming increasingly popular. While complications are uncommon, infections can happen.
This case report describes a patient who developed an infection just days after the procedure. Using point-of-care ultrasound (POCUS), clinicians were able to quickly identify the infected area, locate the PDO threads, and guide their removal in real time. The patient recovered without further complications.
This article explores...
🔹 How POCUS can help identify complications following cosmetic procedures.
🔹 How real-time ultrasound can make procedures more accurate and less invasive.
🔹 The growing role of POCUS in improving the diagnosis and treatment of aesthetic medicine complications.

🫀 Can an ultrasound reveal what traditional hemodynamic measurements might miss?
Venous congestion is often an underrecognized driver of organ dysfunction in patients receiving mechanical circulatory support—but what if we had a simple, bedside tool to detect it earlier? From evaluating complex hemodynamics to guiding clinical decision-making, VExUS has the potential to become an invaluable point-of-care tool for optimizing patient management and improving outcomes.
This article dives into…
🔹How Doppler ultrasound is reshaping the assessment of venous congestion.
🔹 Why VExUS is gaining momentum in the management of patients with mechanical circulatory support.
🔹 The exciting potential of integrating this noninvasive approach into everyday clinical practice.

🫁 Or is it... the liver?
A patient in shock underwent a RUSH exam, where classic lung POCUS findings appeared to point toward pneumonia.
Except they weren't in the lung.
This case is a reminder that familiar ultrasound signs can sometimes be deceptive and that pattern recognition should be paired with careful anatomic assessment.
🔍 Could you identify the real diagnosis?

🔎 Think you know your POCUS scans? What do you see in this image?
This case series highlights a very unique yet deceptive ultrasound artifact that can mimic a life-threatening abdominal aortic dissection — even in otherwise healthy patients.
📍Recognizing this artifact can make all the difference in emergency settings, helping clinicians avoid unnecessary alarm, additional imaging, and potential misdiagnosis. Read through these cases to sharpen your POCUS interpretation skills and stay one step ahead in high-stakes clinical scenarios.

Picture this. A 3.5-year-old previously healthy boy presented with two episodes of non-bilious and non-bloody emesis to the pediatric emergency department six hours following a minor abdominal trauma. The patient had fallen from a standing height onto a seesaw, with the seesaw bar hitting the right side of his abdomen.
What was the first tool taken out of the toolbox? A POCUS FAST exam, later followed by a contrast enhanced ultrasound scan. The CEUS scan revealed a triangular area lacking enhancement in the pre-renal space, indicative of a large hematoma adjacent to the adrenal gland…how did the physicians approach the rest of this case?

🌍 Pediatric TB is a global problem, and access to imaging remains a barrier.
This study presents a standardized mediastinal POCUS protocol to improve detection of TB-related lymphadenopathy in children, particularly in resource-limited settings.
💡 By using mediastinal POCUS to identify lymphadenopathy, clinicians can leverage a radiation-free, bedside tool that’s especially valuable when CT or advanced imaging isn’t available.

A new study in the @pocusjournal shows how you can use color power Doppler to measure blood pressure.
They placed the color Doppler box over the brachial artery and inflated the blood pressure cuff until the Doppler signal disappeared. They then slowly release the cuff until it reappears, mimicking how it is done with a stethoscope.
They compared the pressures to an arterial line, and the results were quite impressive. The Pearson correlation coefficient was 0.95, suggesting that a systolic pressure from this method or from an arterial line can be used interchangeably.
This is an interesting use case for color power Doppler, which can detect very low-velocity flows and is not angle-dependent like color Doppler.
