Preexcitation syndrome or accessory pathway. Because the delta wave in lead I and aVL is positive and in lead aVR is negative.
Right side accessory pathway is preferred, LIII and aVF show Q wave as if inferior infarction so accessory pathway is posteroseptal. Note the short PR and the delta wave in LII, V3, 4, 5, 6.
