GREAT to be LIVE in LA, doing pre-meeting meetings, awaiting the start of USCAP2022. USCAP2020 was one of the last live conferences before the pandemic tornado hit us. Awaiting the arrival of the @UIPathology team and Pathology colleagues from world over. See y’all soon! 😁

A weekend flow case after a long time.... Here is a quick-and-dirty slide (sorry for the expeditious cluttered labeling). History on the slide. What do you think about the red population? #GoWithTheFlow @UIPathology

OK - doing this after a long break. Look at the image and see what you think about the red population. Diagnosis? Don't get to see these often anymore [unlike in the mid-90's when we might see 2-3 cases/week at some centers]. So throwing this one into #PathTwitter. #GoWithTheFlow

Agree. There is variability. Need to use combination of markers and do intracellular kappa/lambda staining to be definitive. In our study of ~100 BM specimens with polytypic/"normal" plasma cells, CD117 was uniformly negative. See: onlinelibrary.wiley.com/doi/… and attached figure.

ONE-TWO PUNCH: I think the image says it all! Second dose done. Feeling highly privileged, proud and grateful 🙏🙏👏👏 So many people collaborated to make this possible. Simple message/plea: Please take the vaccine whenever offered. It's the only way to bring this under control.

The key feature is the predictable MATURATION SPECTRUM reminiscent of normal thymocytes [see arrows in the image]. I find the CD3/CD1a and the CD4/CD8 dotplots most useful. These four markers will differentiate thymocytes from neoplastic T lymphoblasts in (almost) all cases.

#GoWithTheFlow @UIPathology Back to the CD10+ theme [although we don't pay too much attention to this marker in this scenario]. Here is another classic Friday evening urgent call. Young man, anterior mediastinal mass... 1. Are you seeing the tumor cells? 2. Likely diagnosis?
