When we researched (@PeterResteghini) this area we found CSA difference of 2mm ( CSA at level of pisiform minus CSA at pronator quadratus) to be best followed by CSA. WFR was specific but in our study not as sensitive. For me US is first line before NCS for straight forward CTS.
