5/6 Ultimate diagnosis: Legionella pneumonia c/b rhabdomyolysis & cytotoxic corpus callosum lesion😵💫 – both of which can be associated with Legionnaires’ disease. Rx’d 💊 with azithromycin with rapid improvement!
1/3 Continuing our “schema series” (in which IM faculty +/- specialist consultants review dx approaches 🔎 & mx strategies 💊 for CC), Chief Dr. @PatrickHolmanMD led us through an approach to shock with expert insight from beloved pulm crit fellow Dr. Michael Troy 🔥
3/3 Important points!
💥Mx of shock: assess pt stability while giving empiric tx + building a ddx. Keep a framework (shock types vs PROVED?) in mind.
💥Focused exam: 1) doorway 👀 2) perfusion check (🧠 & skin) 3) get access 4) 🔎 volume status 5) organ exams +🫀🫁 POCUS

3/4 @JLabin_MD led @DxRxEdu and our audience along a wild ride culminating in the humbling diagnosis of HIV/AIDS c/b pulmonary TB and disseminated Burkitt’s lymphoma marked by multi-organ failure & TLS 🤯
