Welcome to the receipts desk 🧬 SMID biotech & gene editing. Numbers first, caveat second — and only with receipts. House rules: 1. Hype is welcome. Receipts are mandatory. FDA letters, 8-Ks, trial registries. Not "people familiar with the matter." 2. A press release is the trailer. The filing is the movie. 3. "Up to $2B" is not $2B. Milestones are wishes with a spreadsheet attached. 4. When I'm wrong, I'll say it here. No quiet deletes. 5. Sarcasm is free. Facts aren't negotiable. Moonshot calls are fine. Moonshot calls with zero receipts get roasted. Follow if you want the numbers and the caveat.

Two years on first-line therapy, kidney function barely moved. $TVTX's FILSPARI in newly found IgA nephropathy (SPARTAN, ASN): 75% hit complete proteinuria remission at some point; eGFR down 1.75 by wk 110. Only 12 patients, open-label, no control arm: a signal, not proof.

From 2.5 hours to 15 before blood sugar crashed, after one infusion. $BEAM's base editor BEAM-301 in GSDIa (ESGCT): 2 of 3 at 40 mg finished a 15-hour fast and stopped cornstarch. The third didn't improve. n=6, open-label, 3 Grade 3 infusion reactions. Can 60 mg close the gap?
1/🚨WOW! @BeamTx has presented initial clinical data from its ongoing Phase 1/2 clinical trial evaluating $BEAM - 301 - a liver-targeting lipid-nanoparticle (LNP) formulation of adenine base editing (ABE) editors designed to correct the R83C mutation, as a “one and done” treatment for patients with glycogen storage disease type Ia (GSDIa). The most significant finding in today’s clinical data was that in the 40 mg cohort, two patients have achieved 15 hours of controlled fasting, improvements in glucose control and no longer required starch supplementation following a single administration of BEAM-301. These results strengthen BEAM-301’s potential as a “one and done” Gene Editing treatment for GSDIa patients by replacing the current intensive daily management which is currently required for treating this disease. Another strong indication of Base Editing’s potential as a meaningful therapeutic modality.

Biotech can sit flat for years, then move in a day. $GMAB was $28.29 when I flagged it in Jan 2024 for a 3 year hold list. It closed $57.29 on Oct 28, +103%. Not advice, just an observation. Which pipeline asset matters most now?
Solid 3 Year Hold Time Frame Large Cap #Stock list. Only 1 #Biotech on the list, but IMO it’s a 💎 so good pick $GMAB Genmab 👀 ⬇️

59.1% vs 24.6%. Same trial, sliced after the fact. $IOVA's one-time TIL lifileucel, 2L nonsquamous lung cancer: 17 of 69 responded. KRAS wild-type, HER2 wild-type ever-smokers: 13 of 22. Retrospective cut. POSITILITY tests it prospectively from November. Does the signal hold?

HR 0.50 on survival at the first interim. Genmab's petosemtamab + Keytruda, 1L PD-L1+ head & neck; no median OS yet. $PVLA: rapamycin gel NDA gets priority review, PDUFA by 2/28/27. $BNTC BB-301 in OPMD: 9 of 9 responders, no SAEs, no control. Does 0.50 hold as OS matures?

Oral VK2735 Phase 3 is two 560-patient studies (SUNRISE-1/2), 68 weeks, three doses. Q3 prints after the close today; the Oct raise was ~$548M net. How many Phase 3 quarters does that buy before more dilution?

Q3 is out: $960M cash + short-term investments at Sep 30 (incl. ~$550M net from the September raise), Q3 net loss $183.2M. Net loss isn't cash burn, but at that pace it's roughly 5 quarters. Source: Viking Q3 release.

FDA questioned the pathway the day it took the file. $INO's INO-3107 for HPV airway growths (RRP), PDUFA by Oct 30: single arm, 32 adults, 28% surgery-free in year 1. Filed only for accelerated approval, with Papzimeos already approved. What counts as "meaningful benefit" now?
328
Stopped early, unblinded anyway. $AARD's Phase 3 HERO in Prader-Willi hunger ended at 67 of 90 patients; all efficacy is exploratory. ARD-101 cut HQ-CT "modestly" vs placebo by week 2, then scores slid back off drug. FDA lifted the hold. Can a lower dose keep the signal?
152
$380M raised on a 3.1-point gap. $RAPP sold ~$200M of stock, $150M of 2.25% converts and $30M privately, mainly to fund RAP-219. Its Phase 2 bipolar mania win: YMRS down 15.4 vs 12.3 on placebo at week 3 (p=0.045, n=253). Phase 3 starts 2H27. Does 3.1 points hold in Phase 3?
$RAPP $175M offering priced at $21/share 😬

Two trial deaths, neither blamed on the cells. Artiva quit anyway. $ARTV ends AlloNK: one death tied to fludarabine prep, one unexplained, weak durability. Staff cut 94%. $TEVA bought BioXcel's at-home agitation film, $57.5M upfront. PDUFA by Nov 14. Is fludarabine the risk?

Congrats Cody!!!!
Surprise 1: Anthropic acquired a one-person biotech . . . Surprise 2: that person is me Surprise 3: I’m here to tell you all about it Yes, this is what you think it is; after 6 yrs of increasingly loose anonymity: the Professor Oak reveal post …and yes, it has gotten so boring around here that I’m literally doxxing myself to spice up the timeline ------- Ok, allow me to introduce myself. My name is Cody Tranbarger. I mostly tweet, but occasionally find time to dabble in drug dev & starting/building biotechs. My past lives span the VC, crossover, startup continuum; Prof Oak has been my way of keeping a digital foot in those worlds, despite having my physical feet planted elsewhere. The past 18 mo, ‘elsewhere’ was Mithril Medicine, a co. I founded to develop bsAbs for LAMA2-CMD & other ECM-related genetic diseases. Mithril was born out of hands-on building with @MikeNGladstone, @JasonPaulRhodes, @LifeSciVC, & team @atlasventure. Somehow, I made enough half-decent calls along the way (even a broken clock is right twice a day, eh?) to convince them Mithril/I warranted a small seed. While I can hardly rationalize such an outrageously cavalier bet, I’m grateful for it, & I'd partner with Atlas again in a heartbeat. In typical AV fashion, Mithril’s primordial form was virtual, decentralized, & ultra-lean. This stage tends to be short-lived, but for Mithril, it worked surprisingly well, for surprisingly long, thanks in part to the rapid maturation of the AI tools I used liberally. Eventually, Mithril became a bit of an experiment itself - to find the upper bound of how far it could scale as a single operator + a bunch of Claude (+ a stellar bench of part-time consultants). In parallel, I started spending time with @nc_frey, @an1lam, & @CoefficientBio team; and promptly experienced, for the 1st time, AI at the frontier. Soon thereafter, Mithril stopped looking like an experiment, & started looking like a preview of an entirely new operating model. From that spark came “Pipeline-in-a-Person.” The idea immediately resonated when we shared it here back in April; and it was central to the shared vision that led to Anthropic’s acquisition of Mithril, & my joining the team. Why join Anthropic? Three reasons. (1) For Mithril, the programs continue, with more resources & infrastructure behind them, and as much Claude as I can put to work. (2) For me, the chance to start several more programs, & find out how many one person can actually run in parallel - an almost unprecedented luxury for a curious drug hunter. (3) For our mission, a path to institutionalize & scale this new way of working, then put it in the hands of all of biopharma, all over the world - which, candidly, I find the most energizing. Will AI x Bio cure diseases? Unequivocally yes, imo… but not by the route that gets the most airtime. Rather than timeline acceleration & compression, I see orchestration & parallelization as today's low-hanging fruit. Most drug hunters see a handful of go/no-go calls in a career, & spend entirely too much of the rest trading emails (my inbox can attest). Give them the bandwidth to run several programs at once, and the time to focus on scientific judgment, and I’m willing to bet they’ll make better calls. And at scale, better calls translate to more drugs, higher hit rates, broader patient impact, & likely, a disease or 10 or 100 cured. I couldn't be more excited to build toward that future @AnthropicAI. Finally, a heartfelt thank you to BioTwitter/X. The end of anonymity is bittersweet, but if anyone’s earned a proper introduction, you all have. It’s difficult to overstate how much this app has done for me: a decade of education, plenty of friends, many job opportunities, several mentors, a few investors, even a co-founder (fmr). It was the generosity of this community that inspired me to start posting 6 years ago, after just as many years lurking. I hope I’ve been able to pay it forward, in my own small way. But, to be clear, Prof Oak isn’t going anywhere. Same beat, same themes, and now, a front-row seat to drug development at the frontier. Stay tuned for dispatches from the trenches; and remember... Gotta Cure 'Em All

38% in immunologic remission by week 12. $CLYM's CD19 antibody budoprutug in membranous nephropathy (ASN Kidney Week): 5 of 13 at 200 mg; anti-PLA2R antibodies down 70% on average. No related serious AEs. Proteinuria lags antibodies. Do higher doses move it?
139
Strong response, heavy side-effect bill. $XNCR's XmAb819 in pretreated kidney cancer (ESMO): 45.5% confirmed response at the chosen 400 mcg/kg dose, n=11. Grade 3+ related AEs: 72.1% across expansion doses, led by AST/ALT. Is n=11 enough to pick a Phase 3 dose?
103
Built for tumors the first G12C drugs already failed. Private Frontier Medicines' FMC-376 (ESMO, lung cancer): 4 of 9 responded after a prior KRAS G12C pill; 10 of 18 overall. Median response 7.5 months. Tiny n, no control arm. Does ON+OFF binding beat resistance or delay it?

Phase 2 halved vomiting. Phase 3 cut it 27%. $VNDA's tradipitant blocks NK-1, a brain nausea receptor. Given before one 1 mg semaglutide shot with no usual ramp-up: 43.0% vomited vs 58.5% on placebo (n=306, p=0.006). Does the gap hold with normal slow dosing?
$VNDA Vanda Pharmaceuticals — Phase 3 NEREUS (tradipitant) hit its primary endpoint, any vomiting after semaglutide fell to 43.0% vs 58.5% on placebo, a 27% cut — Vomiting with severe nausea down 35%, no new safety signals — The 60% cut in frequent vomiting and 81% cut in extreme vomiting are post hoc analyses — Plans to meet the FDA and file an sNDA for GLP-1 related vomiting prevention — Risk flag: single 1 mg semaglutide dose, no gradual titration like real use Not financial advice. My earlier VNDA tweet had this wrong. The 27% to 81% was not one range, 27% was the primary result and 81% was the post hoc extreme vomiting number.

61% fully clear skin. Placebo: 0%. $ORKA's IL-23 antibody ORKA-001, Phase 2b psoriasis, week 16, top dose. $MGNX's PD-1/CTLA-4 bispecific at ESMO: 4 of 16 clear-cell gyn cancers responded, mPFS 2.9 mo, grade 3+ side effects in 49%. Does clear skin hold on 1–2 doses a year?

$318M before any patient data. $LYCA, Nobel laureate Carolyn Bertozzi's protein-degrader biotech, priced an upsized Nasdaq IPO. Its two lead drugs (IgE in allergy, Graves' autoantibodies) are in Phase 1; data in 2027. Platform premium before patient data. 2027 reads will test it.
