🔎 Who has actually cured multiple myeloma? Now that IMS/IMWG has proposed a rigorous definition of “cure,” let’s find the patients who may already be there. The bar is high: ≥5 years in CR off therapy, MRD-negative at 10⁻⁶ with ≥4 negative assessments, and negative functional imaging. 🟢 TOTAL THERAPY Some patients remain progression-free 10–20+ years after finite therapy. They may be cured, but these trials predated modern MRD/imaging requirements. 🟣 MASTER 84/118 achieved MRD <10⁻⁶ and entered MRD-SURE. Beautiful proof of principle, but follow-up off therapy is not yet ≥5 years. 🔴 CARTITUDE-1 32/97 alive and progression-free ≥5 years after one cilta-cel infusion, without maintenance. Among 12 intensively followed long-term survivors: ➡️ 12/12 MRD-negative ➡️ MRD negativity reported at 10⁻⁶ at ASCO ➡️ 12/12 PET/CT-negative through ≥5 years These may be our strongest contemporary candidates for documented cure. We need the serial patient-level MRD data to know whether they fulfill every criterion. 🔵 CARTITUDE-2A 10/20 alive and progression-free ≥5 years after one cilta-cel infusion without maintenance. At year 5, 3/3 tested were MRD-negative at 10⁻⁶. So who cured myeloma? Total Therapy may have done it first. CAR-T may allow us to prove it molecularly. MASTER may show us how to design treatment to end in cure.
