ALT JAMA study: Diagnostic yield of transbronchial lung biopsy. 252 males, 248 females, mean age 62.6 years. 500 patients randomized. Cryoprobe: 88.6% diagnostic yield (217/245). Forceps: 78.8% (193/245). Higher yield with 1.1-mm cryoprobe vs 2.0-mm forceps.

Thrilled to see VERITAS out in @NEJM today. Nav bronch noninferior to transthoracic needle biopsy for lung nodules 10-30 mm. Nav bronch made an accurate diagnosis in 79% vs. 73% for TTNB, with pneumothorax requiring chest tube and/or hospitalization in 1% vs. 12%, respectively.

Great work from @KaeleLeonard @seeweilow and the VIPR crew. Bottom line: less than 1% diff b/w diagnostic yield and accuracy when conservative/strict definition used. Don’t need to wait a long f/u period for accuracy before reporting reliable results on diagnostic utility.
This paper is a long time in the making, with a lot of thanks to my mentors @RobJLentz and @MaldonadoFabien, providing evidence to support the recent @atscommunity and @accpchest joint recommendation for a conservative diagnostic yield definition for peripheral pulmonary lesions
