Women remain significantly underrepresented in structural heart trial leadership.
Among 88 trials, women held just 7% of committee seats, chaired 12%, and were first or last author on 14%.
Representation in research leadership matters.

Valve-in-valve or valve-in-ring TMVR - how do outcomes differ?
In our pooled analysis of 1,947 patients:
🔹 30-day mortality was similar
🔹 ViV-TMVR → ↓ LVOT obstruction
🔹 ViV-TMVR → ↓ reintervention
🔹 ViV-TMVR → ↓ acute kidney injury
The takeaway? ViV and ViR are not the same procedure - anatomy matters, and thoughtful patient selection and procedural planning are essential.
Very excited to see this work published and grateful to everyone who contributed!

Excellent commentary by Drs. @AndrewOseran and @prihatha on our recent study in @JACCJournals. Beyond the four foundational pillars of HFrEF therapy, they emphasize policy as the critical “fifth pillar”, bridging the gap between what evidence recommends and what patients actually receive. 💊🏛️🫀 @gcfmd @ATSandhu @SJGreene_md @DOM_UCLA @uclaCVfellows @DOM_UCLA

🤖 Is AI-powered 3D echocardiography truly automated?
Artificial intelligence is transforming 3D echocardiography, but this editorial reminds us that automation is only as good as the images it receives. Without standardized acquisition and expert oversight, AI may deliver precise-looking—but potentially misleading—results.
🔍 Key messages:
✅ Image quality remains the single most important determinant of AI performance. Automation cannot compensate for poor image acquisition or non-standardized datasets.
✅ AI algorithms may suffer from "silent drift"—maintaining the appearance of accuracy while progressively losing reliability when applied to new scanners, patient populations, or disease phenotypes outside their training domain.
✅ In conditions such as hypertrophic cardiomyopathy, inaccurate automated LVEF measurements could directly influence treatment decisions, highlighting the need for phenotype-specific validation.
✅ The authors propose that AI should function as an expert-supervised assistant, supported by standardized acquisition protocols, quality control, and continuous validation—not as a replacement for clinical expertise.
💡 The future of AI in echocardiography is not about replacing experts—it's about combining high-quality image acquisition with intelligent automation to achieve more reliable and reproducible patient care.

Heart research is advancing, but representation still has catching up to do. ❤️🫀
A study led by BCM resident Dr. Sahar Samimi found women hold just 7% of leadership committee seats in interventional structural heart disease trials.

Good summary of A-CLOSE comparing clooidogrel monotherapy to DAPT high ischemic risk patients.
Big lesson: NACE is not a meaningful endpoint when there are tradeoffs.
And unless you're willing to take on mortality risk to avoid minor bleeding, dapt was a lot better.

1) HFmrEF is gone.
Acute HF is gone.
NT-proBNP cut-offs are now age-adjusted.
Everything that matters 🧵👇
2/ 🔄 NEW DEFINITION — two phenotypes only
❤️🩹 HFrEF → LVEF <50% + symptoms/signs
🫀 HFpEF → LVEF ≥50% + symptoms/signs + objective evidence of structural/functional abnormality
❌ HFmrEF eliminated: same aetiology, same pathophysiology, same treatment response as HFrEF

🫀Grateful to see our work on women’s representation in the leadership of interventional structural heart disease trials now published in @JACCJournals, alongside a very thoughtful accompanying editorial by @Drroxmehran, @ditchhaporia and Dr. Puja B. Parikh.
Among 88 high-impact SHD trials, women held just 7% of trial committee positions, with no meaningful improvement over 15 years.

4/ But two important challenges remain: mild PVL and permanent pacemaker implantation rates were high.
At 30 days:
🔹 Mild PVL: 31.1% Evolut & 19.9% Navitor
p-interaction = 0.046
🔹 New PPI: 13.4% & 15.3%
p-interaction = 0.26
So, while clinically significant PVL is now uncommon, further room for refinement remains, as even mild PVL has been associated with bioprosthetic valve failure and increased all-cause mortality.
7/ Take-home message:
Contemporary self-expanding THVs demonstrate:
✅ High device success
✅ Low early mortality
✅ Favorable hemodynamics through 1 year
✅ Low rates of moderate-or-greater PVL
🚨At the same time, mild PVL and PPI remain important opportunities for continued procedural and device refinement.
What’s next? More granular procedural/anatomic data and longer-term follow-up to better understand durability and reintervention rates.
8/ Huge thank you to all of our co-authors for the incredible work that went into bringing together this contemporary pooled analysis of self-expanding TAVR outcomes, and especially to Dr. @SachinGoelMD for his invaluable mentorship and guidance throughout.
And a special shoutout to @DrShawnShaji, @praeformed, and @RahulSark13, who put an enormous amount of work into this project!
