Our volunteer fireman do just that - volunteer/fight fires to save their communities and they don’t want/expect to be paid for what they do. No one would have expected the call on them to last so long. Compensate them appropriately. $200 per day is insulting capping it is worse.

Food for thought: until tracheal intubation was introduced, oesophagectomy intubation never happened! There are other consequences of gaining to secure the airway of course. #WAMMsterdam

If preoxygenation is performed in emergency patients with high flow 02 for 3 minutes, with no measurement of Fe02 to determine adequacy of denitrogenation, quite possibly supine so with limited lung volumes, why are we surprised that patients desaturate during intubation?

Cricoid pressure or not? Nobody knows how often it has prevented regurgitation and it can always be removed if felt to hinder intubation. #WAMMsterdam

Head down position may reduce the incidence of aspiration but increase passive regurgitation. Head up may reduce passive regurgitation but increase aspiration should regurgitation occur. Hmmm, which to choose? I’ll take the one that improves oxygenation! #WAMMsterdam

#WAMMsterdam we need to move away from the simple statement “difficult airway” to using a descriptor “difficult airway because.....” ( face mask seal, oral access, laryngeal view, can see but couldn’t get there etc)
