Kudos to @vipuljairath for the tenacity to answer the difficult questions. Endoscopic healing all the way to 0 results in best UC outcomes & is associated with with histo remission too
The #VERDICT is out for T2T in UC @my_ueg
🔺Vedo w step-up if target not achieved
🔑Take home message
⬇️UC complication if CS-free sympt+endo+histo remission vs sympt remission alone
🔑Endo remission = endo+histo remission
⚠️Our 🎯should be Mayo 0/ not Mayo 1 @vipuljairath

Taking it up a notch w #STRIDE-III @my_ueg
🔺What changes from STRIDE-II to III 👇🏽
🔺Separate actionable 🎯 which need Rx change after optimization vs patient centered 🎯
⏳Timeline to achieve 🎯
⚠️Avoid under & over treatment

PSA from your friendly endoscopist who’s been doing this x 25 years.
Please swap your irrigator from right to left- every other case or every other day…
🤔Then you too can avoid:
Shoe size increasing from 8->9 1/2W
Foot surgery on a random Wednesday

If the generator shapes the tissue effect ..why do APC, bipolar probes, and hemostatic forceps behave so differently?
Three devices. Three different physics.
Not all thermal hemostatic devices work the same way.
Although APC, bipolar probes, and hemostatic forceps all achieve hemostasis, they differ in how electrical energy is delivered, whether tissue is compressed, and the type of bleeding they’re designed to treat.
🟠 APC delivers non-contact monopolar energy through ionized argon gas, making it ideal for broad, superficial lesions.
🔵 Bipolar probes combine mechanical compression with localized bipolar current (coaptive coagulation), making them well suited for visible vessels and focal ulcer bleeding.
🟢 Hemostatic forceps first grasp the vessel, then deliver monopolar coagulation, allowing precise treatment of targeted bleeding, particularly during EMR, ESD, or refractory bleeding.
The goal isn’t to memorize devices. It’s to understand the physics behind them.
Match the device to the lesion, not habit.

Let’s talk about pivoting @BMoshiree @ScrubsNHeels #ScrubsNHeels2026
• Reflect on what actually fulfills you
• Audit your transferable skills
• Explore before you exit
• Upskill in one focused area
• Build a timeline, not a leap
• Find mentors and community
The hardest part isn’t logistics.
It’s identity.
Pivoting feels hard because we anchor to: “I’m a clinician.”
Instead: “I have clinical expertise + ____ skills.”
That’s where options open up.
Pivoting isn’t failure.
It’s alignments.
Pivoting is the new path for career growth 🔄 @BMoshiree
💎Spot the signals that it’s time 👀
💎Normalize pivoting as growth 💡
💎”Pivoting isn’t a failure, it’s an informed evolution.”
Careers evolve - so should we✨

@tommasoparigi at #ECCO2026 about combination therapy future, one option is bispecifics (or trispecific!) for IBD
Not multiple drugs, but multiple targets within one drug.

ECCO 2026 has been the best ECCO congress in several years. Biggest surprise for me however was that appendectomy is being recommended for therapy-refractory UC in the upcoming ECCO UC guidelines. Has anyone recommended this for a patient yet?
