The global HIV response is a UN success: infections down 61%, deaths down 70%, millions of lives saved.
Yet at the High‑Level Meeting on HIV/AIDS, I warned that progress is stalling. Funding is shrinking, prevention is being scaled back, and stigma is rising.
The end of AIDS is within reach. We cannot let it slip from our hands.

Despite progress, millions still lack access to HIV prevention and treatment services.
Ending AIDS means empowering communities, investing in prevention and expanding access to treatment for all people.
We have the power to transform lives and #endAIDS once and for all.

Today is #WorldAIDSDay.
The HIV response has always shown what humanity can achieve when we stand united.
Global collective action has saved millions of lives, but shrinking funding now puts this progress at risk.
Leaders must recommit to ending AIDS and saving lives.

#GlobalHealth has been very much anchored in global solidarity - countries agreeing to work together to deliver on health. And it's been driven very much by aid for the developing countries.
But now we're seeing that aid tumbling very fast — that suddenness, that rapid decline, is costing lives.
Let's be clear about that: people are dying.
So the first thing we need to do is to organize a planned, a responsible transition from a model of aid.
We've seen the vulnerabilities of aid, and now we want to go away from that. But we must do it in an orderly way that's not costing lives.

In those countries that were overdependent on aid, what happened in the area of HIV, was the creation of parallel structures where this money for HIV was driven, while the rest of the health systems were crumbling. We must solve that.
We must integrate, build one health system that addresses vertical diseases—one system that delivers cost effectively. These are things we are doing, we @UNAIDS are working with countries.
But first and foremost, we must do it with a plan that's orderly, that's responsible, and that doesn't cost lives. It shouldn't be a sudden retreat, it should be a planned transition.

Aid needs to stay there to keep as a scaffolding as we as we transition. In transitioning, we must solve the problems of the old model — we must not bring back the problems of the old model. One of them was that overreliance on external assistance, and within that overreliance, on a handful of donors of aid.
In the area of HIV, when the US paused their development assistance in February, in the whole global response on the HIV/AIDS, the 🇺🇸 was shouldering 73% of the total burden of development assistance, just one country. The rest of the world was shouldering the 27%. That wasn't right.
So that overdependence on one country, in a few countries, an overdependence of the developing countries, we have to solve it.
