Pay hospitals more if “malnutrition” is in the chart. Hospitals hire teams to comb through patient charts and harass doctors into putting “malnutrition” in the clinical notes. More people are suddenly dying with “malnutrition” as a diagnosis. This is why most research based on administrative codes is total garbage.
Has America had a rapid and sudden rise in deaths due to malnutrition? This has become a common anti-American talking point, but is it true? Well, that is what the death certificates say, but that's just because we started writing them differently!

Nope, I’ve had people talk with me about this article and they are literally like, “how could doctors do that!?!”.

Ms. Weil, explaining the distinction inside a paywalled article doesn’t fix a misleading headline. You acknowledge this concerns hospital-owned practices, yet the headline suggests physicians are pretending their offices are hospitals to inflate charges. Hospital ownership is central to the story not the physicians. It is definitely misleading and implies an agenda. The Washington Post’s standards say: “No story is fair if it consciously or unconsciously misleads or even deceives the reader.” As an independent physician, I support scrutiny of hospital facility fees. But the headline should clearly identify hospital ownership so readers know who is responsible. @juliezweil @washingtonpost
Thanks for the feedback. I believe it's very clear in the article that this is entirely about the fees charged by hospital-owned medical practices, as opposed to independent physicians.

Encouraging to see this moving forward. Patient access and safety aren’t competing goals. Licensed pharmacies, verified quality, and appropriate oversight are the path toward access patients can trust. Looking forward to the final policy.
WE ARE CLOSE!!! The federal plan to let pharmacies compound BPC-157 and other peptides is at the White House!!! New: it reportedly requires purity certification and side-effect reporting. And a safety group has petitioned to kill it. What we know, and what hasn't changed 🧵

Hey @washingtonpost This the correct headline. But we know, if you print this, you’ll lose your ad dollars. Eh @JeffBezos ???
The title should be, "When hospitals pretend to be doctors, you pay 10X more".

Important point. Under the ACA, the business model of US health insurance companies is to skim off 15% of total healthcare spend. They are therefore incentivized to maximize the amount of healthcare spend that goes through the insurance system and to see total healthcare spend to increase rather than decrease.
Why would insurance companies want premiums to go UP? CEO and founder of CrowdHealth, Andy Schoonover (@SchoonoverAndy), explains how perverse incentive structures disincentivize big medical insurance companies from reducing costs.

The shell game he’s describing begins and ends with government. Medicare’s government-administered reimbursement system allows hospitals to collect separate facility payments for outpatient services while reimbursing independent physician practices at lower rates for many of the same services. Hospitals and other savvy corporations can acquire or establish outpatient practices and, through hospital affiliation and qualifying facility designations, become eligible for additional payments unavailable to independent practices. These government-created financial incentives reward consolidation, favor large hospital systems, and undermine smaller independent practices. Yet it’s so much sexier to blame the greedy doctor while the benevolent government bureaucrats and their corporate cronies get off scot-free, bonuses in hand, for supposedly making the world a better place. The problem isn’t your typical small private practice. It’s the government-designed system and the corporations that profit from it.
Doctors’ offices are charging patients 10 times more by claiming to be hospitals
Even when patients never step into a hospital, they may be charged hospital “facility fees,” and their care may be billed at higher rates. washingtonpost.com/business/…

Son: Dad, you're always calling things iconic from the '80s. What does that even mean? Dad: Son, it means 40 years later, a scene can still give you the exact same feeling it did the first time you watched it. This is one of those moments.

Joe Rogan just exposed medicine's dirtiest secret: "peer reviewers" never see the data. ROGAN: "if it's peer-reviewed, the scientists do not get to look at the actual data... that's one of the reasons that they can say crazy things like it's 100% effective." And the proof? Vioxx. Merck removed three heart attacks from a Vioxx publication, the New England Journal published it anyway, and the drug is estimated to have killed 120,000 people. Here's how the rest of the machine works. 🧵

new #preprint! @niroshajmurugan Does the brain know what's going on in a limb amputation wound - whether there's a pro-regenerative intervention or not? Let's look at its gene expression levels, with and without our biodome: "Limb-localized induction of regeneration reshapes brain and amputation-site transcriptional states in adult Xenopus laevis" biorxiv.org/content/10.64898… yes, it knows. Also, lots of interesting transcriptional responses both in the limb and in the brain. Plenty of leads for subsequent investigation.
