Post-cholecystectomy biliary pain isn’t always the remnant GB.
40F. Subtotal cholecystectomy 2018. Recurrent postprandial biliary pain. Residual GB — no stones. LFTs normal.
MRCP: ~16 mm distal intramural cystic dilatation → type III choledochocele (Sarris–Tsang B).
ERCP:
• Cholangiogram — 2 cm diverticulum, delayed contrast clearance
• Needle-knife papillotomy + extended sphincterotomy
• Unroofing / marsupialization of the cyst
Post-unroofing cholangiogram: cyst no longer opacifies. Free bile flow. Uneventful.
Treat the ampulla first when the remnant is stone-free.
