🫀 Are we measuring pericardial effusions the wrong way? We routinely classify pericardial effusions as small, moderate, or large based on linear measurements. But does a single measurement in millimeters accurately represent the actual volume of pericardial fluid? Study in @JournalASEcho challenges this onlinejase.com/article/S0894… 🔬 245 patients undergoing pericardiocentesis Median drained volume: 500 mL (range 50–4,300 mL) 76.7% had cardiac tamponade Actual drained volume served as the reference standard. 📊 How did different echo methods perform? Between end diastolic thickness, plax and PSAx area- apical 4 chamber Simpson’s volumetric estimation performed the best ✅ Accuracy: 80.4% vs 49.4% Perhaps it is time to rethink how we quantify pericardial effusions! 💡 if volumetric assessment is substantially more accurate, should we move from millimeters to milliliters in routine echocardiography? And more importantly, would this change clinical decision-making—or simply improve the precision of our reports? #EchoFirst #CardioTwitter #CVImaging

This @CASEfromASE report describes a patient who presented with a large, benign hemorrhagic pericardial effusion. 7 weeks later, a 6.7 cm intrapericardial mass adjacent to the RA with disseminated pulmonary, hepatic, and cerebral metastases was revealed. bit.ly/4rCPfVE

Paradoxical septal motion helps distinguish right ventricular volume overload from pressure overload on echocardiography. In the parasternal short-axis view: 🟦RV volume overload (e.g., severe tricuspid regurgitation): - The RV is dilated. - The interventricular septum appears flattened during diastole. - During systole, the septum returns to a normal contour, and the LV appears round. ⬛ RV pressure overload (e.g., pulmonary hypertension): - The RV is also dilated. - The septum remains flattened during both diastole and systole. - This creates the classic D-shaped left ventricle, a hallmark of increased RV pressure. A simple way to remember it: 🔻Diastolic septal flattening = RV volume overload 🔻Persistent systolic + diastolic septal flattening (D-sign) = RV pressure overload

#echofirst ✅ Time to close. 🙏 for comments ✅ Unusual PW Doppler from PV showing a brief reversal prior to the usual P wave related reversal (Ar) ✅ This is not an artifact since it was reproducible, gain setting was OK etc ⬇️ ✅ During later part of diastasis here there was a brief period when LV diastolic pressure exceeded LA pressure ✅ Is this an unusual finding indicative of ⬇️ diastolic LV compliance? Stiff LA? @echo_batman @argulian @EchoSoliman @strain_rate @iamritu @BryanTanMD @HeartOTXHeartMD @purviparwani @JaeKOh2 @SNagueh

Most echocardiography laboratories have adopted the 2016 diastolic guidelines. With the release of the 2025 update, an important question arises: What are the key conceptual differences between the 2016 and 2025 approaches to diagnosing diastolic dysfunction? academic.oup.com/ehjcimaging… @JGrapsa
