It isn't easy to keep retelling Tom's story but I want to make sure that what happened to him doesn't get forgotten and that he doesn't just become a statistic.
Thank you to @SusannaGalton for this beautifully written piece in the Telegraph.

Richards, GC. 2024. The Preventable Deaths Tracker. preventabledeathstracker.net @Richards_G_C
I don’t believe that there is enough power to make organisations accountable for their negligence, so I’m asking for your help.
Please share this post to raise awareness and make some noise about it —because no family should have to go through this.

Today marks the deadline for two organisations to share the measures they’ve implemented to prevent similar negligence in the future.
What do I hope to achieve by posting this? These aren’t just statistics. They represent families who are left waiting for action.
Yet, there is no system to ensure compliance with this legal requirement.
Last year, my partner died due to preventable medical negligence, leaving me and our newborn daughter, Harper, behind. A Prevention of Future Deaths report was ordered at the inquest—56 days ago.
- 1 in 5 deaths in the UK are avoidable.
- 1,249 future death reports are overdue a response.
- In the past year, less than half (~42%) received a response on time.
By law, organisations must respond within 56 days, outlining actions to prevent similar incidents.
According to the Preventable Deaths Tracker Database, an average of 470 coroners' Prevention of Future Deaths (PFDs) reports are published each year.
The tracker reveals some deeply troubling issues 🧵

I think we can all agree that KPIs are vital in #digitalPR but could we be doing a better job at setting them?
With so much data at our fingertips, why are we still only setting targets around links and domain authority?
I know we can do better 👇

For the last ~year or so I have been logging my little girl’s sleep using @huckleberrycare.
For her first birthday yesterday they sent me a cool data-vis 🤓 pretty cool to see it like this.
Blue is sleep, white is awake and each circle represents 24hrs.
